Yirmi sekiz hafta ve altındaki preterm bebeklerde profilaktik ve selektif surfaktan uygulaması: tek merkez deneyimi

Abstract

AMAÇ: Yüksek riskli prematür bebeklerde profilaktik veya erken surfaktan uygulamasının geç kurtarma tedavisine göre daha iyi sonuçları olduğunun bilinmesine rağmen farklı gebelik haftalarına sahip bebeklerde surfaktan uygulamak için en iyi zamanın ne olduğu sorusunun cevabı tam olarak belirlenmiş değildir. Bu çalışmada doğum salonunda profilaktik surfaktan uygulamasının pnömotoraks, intraventriküler kanama (İVK), patent duktus arteriozus (PDA), kronik akciğer hastalığı (KAH), nekrotizan enterokolit (NEK) ve mortalite sıklığı ile mekanik ventilasyon ve hastanede kalış süreleri üzerindeki etkisinin geriye dönük değerlendirilmesi amaçlanmıştır.YÖNTEMLER: Ocak 2006-Aralık 2010 yılları arasında Gazi Üniversitesi Tıp Fakültesi Hastanesi Kadın Doğum Ünitesinde doğan ve surfaktan uygulanan 28 hafta ve altındaki preterm bebeklerin dosyaları ve hastane otomasyon bilgileri geriye dönük olarak incelendi. Surfaktan uygulama zamanına göre bebekler iki gruba ayrıldı. İlk 15 dakikada surfaktan uygulananlar profilaktik surfaktan grubunu, RDS saptandıktan sonra tedavi uygulananlar ise selektif surfaktan grubunu oluşturdu.BULGULAR: Profilaktik surfaktan (n=24) ve selektif surfaktan grupları (n=46) arasında IVK, PDA, KAH, NEK sıklığı ile mekanik ventilasyon ve hastanede kalma süreleri açısından istatiksel olarak anlamlı bir farklılık saptanmadı. Pnömotoraks insidansı profilaktik surfaktan uygulanan grupta anlamlı derecede daha düşük bulundu (p=0.02).SONUÇ: Yirmisekiz hafta ve altındaki preterm bebeklerde profilaktik surfaktan uygulaması yalnızca pnömotoraks insidansını anlamlı ölçüde azaltmaktadır. Ancak diğer morbidite durumlarında ve mortalite oranlarında anlamlı bir iyileşme sağlamamaktadır.

Prophylactic and selective surfactant administration in preterm infants with 28 weeks and lower: a single center experience

Abstract

OBJECTIVE: Although recent randomized and controlled trials have demonstrated that prophylactic or early surfactant administrations have better outcomes in high risk preterm infants compared to delayed or rescue surfactant treatment, there is no definitive answer for the question of best timing of surfactant application at different gestational ages. In this retrospective study, our goal is to retrospectively determine the effect of delivery room surfactant treatment on pneumothorax, intraventricular bleeding (IVK), patent ductus arteriosus (PDA), chronic lung disease (CLD), necrotizing enterocolitis (NEC), mortality and the duration of mechanical ventilation, hospitalization of each patient.METHODS: Preterm infants with a gestational age 28 weeks and lower who received surfactant between January 2006 and December 2010 were included in this study. Preterm infants were placed into prophylactic group (infants who received surfactant within fifteen minutes after birth) or selective group (infants who received surfactant therapy for established RDS) which were determined according to the time of surfactant replacement therapy. RESULTS: There were 24 patients and 46 patients in prophylactic group and selective group respectively. Although the number of patients with pneumothorax was significantly lower in prophylactic group (p=0.02), there was no significant difference between two groups for IVK, PDA, CLD, NEC, the duration of mechanical ventilation and hospitalization and mortality rates.CONCLUSION: The prophylactic administration of surfactant decreased the incidence of pneumothorax significantly. However, there was no significant difference in mortality rates and neonatal outcomes between two groups. We need further studies to determine the best timing of surfactant replacement.
