Lithium-induced acneiform eruptions

Abstract

Lithium is the drug of choice in the treatment of bipolar mood disorders. Lithium, which causes neutrophilic infiltration, can have negative consequences on the skin. Dermatological side effects such as acne, psoriasis, hair loss, thinning hair, itching, folliculitis and maculopapular rash may occur during lithium use. Acne, especially due to lithium use, is one of the most frequently reported dermatological side effects. However, these side effects may not be questioned or noticed in clinical practice. Regarding the diagnosis of lithium-induced acne development, the history of drug intake, the sudden onset and an unusual age of onset, as well as the initiation of lithium therapy, the drug dose, the duration of the treatment, the absence of other triggering factors, and the clinical relationship between the administration of the drug and the onset of acne should be examined in detail. In our study, a 27-year-old female patient with bipolar disorder using lithium treatment and who developed widespread acne as a result of increasing the drug dose was presented in the light of literature findings. Acne development due to lithium use may be a problem for patients and may affect treatment compliance. Clinicians are advised to carefully monitor dermatological side effects in patients receiving lithium therapy.

Lityum kullanımına bağlı gelişenakneiform ilaç erüpsiyonu

Abstract

Lityum bipolar duygudurum bozukluklarının tedavisinde en çok tercih edilen ilaçtır. Nötrofilik infiltrasyona neden olan lityum, deri üzerinde olumsuz sonuçlara neden olabilir. Lityum kullanımı sırasında akne, psöriyazis, saç dökülmesi, saçlarda incelme, kaşıntı, folikülit ve makülopapüler döküntü gibi dermatolojik yan etkiler ortaya çıkabilir. Özellikle lityum kullanımına bağlı gelişen akne, en sık bildirilen dermatolojik yan etkilerin başında gelir. Ancak, klinik uygulamalarda bu yan etkiler sorgulanmayabilir ya da fark edilmeyebilir. Lityuma bağlı akne gelişiminin tanısı ile ilgili olarak ilaç alım öyküsü, ani başlangıç ve olağandışı bir başlangıç yaşının yanı sıra lityum tedavisinin başlangıcı, ilaç dozu, tedavi süresi, diğer tetikleyici faktörlerin yokluğu ve ilacın verilmesi ile akne başlangıcı arasındaki klinik ilişkinin ayrıntılı olarak incelenmesi gerekir. Çalışmamızda bipolar bozukluk tanısı ile lityum tedavisi kullanan ve ilaç dozunun arttırılması sonucu yaygın akne gelişimi izlenen 27 yaşındaki kadın hasta literatür bulguları eşliğinde sunulmuştur. Lityum kullanımına bağlı akne gelişimi hastalar için sorun olabilir ve tedavi uyumunu etkileyebilir. Klinisyenlerin lityum tedavisi alan hastalarda dermatolojik yan etkileri dikkatle izlenmeleri önerilir.
