Sol Femoral Cisim Kırığı Olan Bir Hastada Sağ Patella Kırığı

Abstract

Patella kırığı dize dik olarak gelen travmalar sonrası sık görülür. 20 ile 50 yaş arasında erkeklerde görülen kırıkların yaklaşık %1 inde görülür. Ama femur şaft kırığı beraberinde karşı taraf patella kığı görilmesi nadirdir ve kolaylıkla atklanabilir.
Olgumuzda araç kazası sonrası acil servise getirlen 24 yaşındaki erkek hasta sunulmuştur. Hastadan alınan anamneze göre, emniyet kemeri takmadan sürücünün yan tarafında önde oturan kazazedenin bulunduğu araç öndeki kamyona arkadan çarpmış. Sol bacak ağrılıydı, şişlik ve deformite kolayca farkediliyordu. Daha ayrıntılı incelendiğinde hasta sağ dizinde de hafif bir ağrı tarifliyordu. Hastanın sol kalça ve femur grafisinde femur orta şaft kırığı mevcuttu. Bununla beraber sağ diz grafisinde patellanın orta hattan ayrıldığı ve yaklaşık 8 mm açıklık olan parçalı kırık tespit edildi.
Klinisyenler yüksek enerjili travmalarda bu tip küçük kırıkların da eşlik edebileceğini düşünmelidirler.

Right Patellar Fracture in a Patient with Left Femoral Body Fracture

Abstract

A patellar fracture is one of the most common knee injuries, usually, after direct trauma to the knee. It represents nearly 1% of all fractures common in those ages between 20-50 years and males. The coincidence of patellar fracture with femoral body fracture in the opposite leg is rather low frequency; moreover, it can be missed out unluckily.
In this case, 24 years old young man is brought to the emergency department after a car accident. According to the patient’s claim, he sat on the next seat of the driver without a seatbelt, and the car crashed to a truck from behind. The deformed, swollen, and painful left leg is clearly noticed, and also the patient is complaining about mild pain on the right knee in a secondary survey. Standard anteroposterior and lateral radiographic evaluation of the left hip and thigh shows that midshaft fracture in the left femoral bone. Besides the broken left femoral body, X-ray imaging of the right knee demonstrates that the right patella separates approximately 8mm from the transverse line, multiple fragmental fractures in the upper part of the broken segment, and displacement of the articular face.
Clinicians must remind this kind of accompanied minor fractures during the high-energy trauma management.
