Abstract

Tetanos, Clostridium
tetani’nin ürettiği ekzotoksinlerin neden olduğu, nadir ve ölümcül
seyredebilen bir hastalıktır. Çocukluk çağının hiçbir aşısı yapılmamış olan ve
mülteci olarak ükemize gelen yedi yaş erkek hasta tüm vücutta kasılma ve ağrı
şikâyeti ile başvurdu. Alınan anamnezde bir hafta önce düşme sonrası ayağına
paslı çivi batma öyküsü olduğu ve sonrasında herhangi bir sağlık kuruluşuna
başvurmadığı öğrenildi. Hastaya tetanos tanısı konuldu ve çocuk yoğun bakım
ünitesinde izleme alındı. Hastaya tetanos aşısı ve tetanos immünglobulin
yapıldı. Penisilin tedavisi başlandı. Yara yeri debride edildi. Takibinde
kasılmaları artan ve solunum sıkıntısı gelişen hasta entübe edildi. Sonrasında
arrest olan hastaya kardiyopulmoner resusitasyon uygulandı. Hastanın izleminde
akut böbrek yetmezliği ve dissemine intravasküler
koagülasyon gelişmesi üzerine çoklu organ yetmezliği düşünülen hasta takibinde exitus
oldu. Sonuç olarak, görülme sıklığındaki artış nedeni ile özellikle yaralanmalar
sonrası santral sinir sistemi bulguları ile başvuran hastalarda aşı öyküsü
mutlaka sorgulanmalı ve ayırıcı tanıda tetanos düşünülmelidir.

Tetanus in an Unvaccinated Refugee

Abstract

Tetanus is a rare and lethal disease
that results from exposure to exotoxins produced by Clostridium tetani. A seven year-old-male refugee who had not been
vaccinated in early childhood was admitted to the hospital with generalized muscle
spasm and pain. The anamnesis taken from the patient revealed that he had
suffered from nail injury after falling down a week ago and didn’t take any
medical care. He was diagnosed with tetanus and admitted to the intensive care
unit. He was given tetanus vaccine and tetanus immunoglobulins. Peniciline
treatment was started. The wound was surgically debrided. The patient was intubated due to increased muscle
spasms and respiratory distress. After that the patient who went into cardiac
arrest was necessitated cardiopulmonary resuscitation. The patient who
developed acute renal failure and disseminated intravascular coagulation was thought
to have multiple organ failure. And the patient later died during the follow up.  As a result; due to the increased incidence,
the patients who have presented with symptoms of central nervous system,
especially after injuries, must be inquired about the vaccination history and
tetanus must be taken into consideration in the differential diagnosis.
