Koroner Arter Perforasyonu Gelişiminde Nötrofil-Lenfosit Oranının Rolü

Abstract

Amaç: Koroner arter perforasyonu (KAP), peruktan koroner girişim (PKG) sırasında seyrek olarak rastlanan ancak ölümcül sonuçları olabilen bir komplikasyondur. Nötrofil lenfosit oranı (NLO), birçok kardiyovasküler hastalığın şiddeti ve prognozu ile ilişkili olduğu bilinen yeni bir enflamatuar belirteçtir. KAP gelişimi ile NLO ve kırmızı hücre dağılım genişliği (RDW) ilişkisini araştırmayı hedefledik.
Gereç ve Yöntemler: 2012-2018 yıllarında, iki farklı merkezde PKG yapılan toplam 15199 hasta geriye yönelik olarak tarandı. Bu enine kesit gözlemsel retrospektif çalışmaya PKG sırasında koroner perofrasyon gelişen 50 hasta ve koroner perforasyon gelişmemiş 100 hasta dahil edildi. Tam kan sayımı parametreleri bu iki grup arasında karşılaştırıldı. NLO, nötrofil sayısının lenfosit sayısına oranı olarak hesaplandı.
Bulgular: PKG yapılan 15199 hastanın 50'sinde (% 0.32)( ortalama yaş 63,2 ± 12,2, % 64 erkek) koroner perforasyon meydana geldi. Perforasyonların çoğu sol ön inen arterde (LAD) (n = 27,% 54), ardından sağ koroner arterde (RCA) (n = 13,% 26) izlendi. En sık Ellis tip II KAP(%52) görülürken, başlıca perforasyon nedeni balon anjiyoplastiydi (%42). NLO (medyan 3,3 ( 4,0)'e karşı medyan 2,3 (1,6), p<0.001) ve RDW (medyan 14,2 ( 2,0)'e karşı medyan 13,5 (1,3), p=0.002) KAP grubunda anlamlı olarak daha yüksekti.
Sonuç: Perkutan koroner girişim yapılan hastalarda yüksek NLO ve RDW seviyeleri koroner arterde perforasyon ile ilişkilidir.
