Perioperatif dönemde görülen istem dışı hipotermi, santral vücut ısısının 35°C’nin altına düşmesi olarak tanımlanır ve uygulanan cerrahi işleme göre değişmekle birlikte, insidansı %17–90 arasında bildirilmiştir. Hipotermi; kanama miktarında artışa, yara iyileşmesinde gecikmeye ve kardiyak arrest gibi komplikasyonlara yol açabilir. Anestezi altında başlangıçta 0,5–1,5°C arasında değişen ısı kayıpları, operasyon süresi uzadıkça 6°C’ye kadar ulaşabilir. Bu olgu sunumunda, uzun süren bir free flep cerrahisi sonrası hipotermi gelişen bir hastada ekstübasyon aşamasında ortaya çıkan ventriküler fibrilasyon (VF) ve uygulanan tedavi süreci tartışılmıştır.
Unintentional perioperative hypothermia is defined as a decrease in core body temperature below 35°C, and its incidence varies between 17–90% depending on the type and duration of the surgical procedure. Hypothermia may lead to increased blood loss, delayed wound healing, and serious complications such as cardiac arrest. Under general anesthesia, an initial temperature drop of 0.5–1.5°C may occur, and with prolonged surgeries, this decrease can reach up to 6°C. In this report, we present a case of ventricular fibrillation (VF) that developed during the extubation phase of a patient who became hypothermic after a prolonged free flap surgery, and we discuss the treatment approach in light of current evidence.
