Background: Toll-like receptor 2 (TLR-2) plays role in the immune response of mycobacteria. Interleukin-1(IL-1) is a cytokine that plays a role in T cell activation and effect with TNF. Interleukin-1 receptor antagonist(IL-1RA) is a natural inhibitor of IL-1. The aim of this study was to demonstrate the relationship between tuberculosis and Arg753Gln mutation in the TLR-2 gene and the effect of IL-1RA polymorphism, which is claimed to increase the susceptibility to tuberculosis in several studies.

Materials and Methods: We compared mutation analysis of 119 consecutive patients with active tuberculosis or tuberculous pleurisy and 83 ethnically matched healthy volunteers.

Results: GG frequency was 97.48 % and GA frequency was 2.52% in the tuberculosis group. We found no statistically significant risk for the GA genotype and IL-1RA alleles. Allele-2 polymorphism correlation with reduced delayed-type hypersensitivity response to PPD (Purified protein derivative) was not statistically significant in our series (p=0.084).

Conclusion: The results of this study do not support the hypothesis that claims that there is a relationship between Arg753Gln mutation and the risk of developing tuberculosis. The results of the present study do not support the presence of a relationship between IL-1RA polymorphism and the development and course of tuberculosis.

Amaç: Tool-like reseptör 2 (TLR-2) mikobakteri immun cevabında rol oynamaktadır. İnterlökin-1 (IL-1) T hücre aktivasyonunda rol alan bir sitokindir ve TNF’yi etkiler. Interlökin-1 reseptör antagonisti (IL-1RA) IL-1’in doğal inhibitörüdür. Çalışmamızın amacı tüberküloz ile TLR-2 genindeki Arg753Gln mutasyonu ve IL-1RA polimorfizminin etkilerini araştırmaktır.

Materyal ve metod: 119 aktif tüberküloz hastası veya tüberküloz plörezisi bulunan çalışma grubumuzu 83 sağlıklı gönüllü ile karşılaştırdık.

Sonuçlar: Tüberküloz grubunda GG frekansı %97,48 ve GA frekansı %2,52 idi. GA genotipinde ve IL-1RA alellerinde istatistiksel anlamlılık saptanmadı. PPD (Purified protein derivative)’ye gecikmiş tipte yanıtta korele olan Alel-2 polimorfizmi serimizde istatistiksel olarak anlamlı saptanmadı (p=0.084).

Tartışma: Çalışma sonuçlarımız Arg753Gln mutasyonu ve tüberküloz gelişimi arasındaki hipotezi desteklemedi. Mevcut çalışma sonuçları IL-1RA polimorfizmi ve tüberküloz gelişimi arasındaki ilişiyi destememektedir.
