AB  - Amaç: Bu çalışmanın amacı, sağlık sisteminin finansman ve hizmete erişim ile ilgili önemli bir bileşeni olan sağlık sigortası olgusu konusunda, okuryazarlık düzeyinin ölçümüne yönelik bir ölçüm aracı ve metodolojisi geliştirmektir.Tasarım/Yöntem: Çalışmada ölçek geliştirme süreci uygulanmıştır. Türkiye sağlık sistemi finansmanının özellikleri, literatürdeki çalışmalar ve uzman görüşleri çerçevesinde, ölçüme yönelik madde havuzu oluşturulmuş ve ölçüm yapısı tasarlanmıştır. İki ayrı aşamada toplanan verilerde toplam 170 katılımcı yer almıştır. Oluşturulan model, toplanan veriler çerçevesinde, içerik, yapı geçerliliği, iç tutarlılık ve zamana göre geçerlilik analizlerine tabi tutulmuştur.Bulgular: Açımlayıcı faktör analizi sonucunda oluşturulan dört faktörlü yapının, doğrulayıcı faktör analizi sonucunda, faktör yükü ve uyum indeksleri açısından güvenilir ve geçerli olduğu görülmüştür. Sosyal sağlık sigortası boyutunun “genel bilgi düzeyi” ve “bilgi edinebilme yeteneği” alt boyutları ile, özel sağlık sigortası boyutunun ise “kullanabilme yetkinliği” ve “edinebilme/satın alabilme yetkinliği” alt boyutları ile değerlendirilebileceği tespit edilmiştir.Sınırlılıklar: İlgili sürecin farklı sosyal ve ekonomik özelliklere sahip olan bireyler ile yüz yüze yapılabilecek nitel görüşmelerle iyileştirilmesi mümkündür. Bu çalışmada yalnızca uzman görüşü alınmış olduğu için toplum tabanlı bir madde havuzu geliştirme süreci mümkün olmamıştır.Özgünlük/Değer: Sağlık sigortası okuryazarlığının ölçümü konusunda bir araç sunan bu çalışma, sosyal sağlık sigortacılığının temel finansman yöntemi olduğu bir sağlık sisteminde ilk olma özelliği taşımaktadır. Uluslararası literatürde tamamen özel sağlık sigortacılığı üzerine yoğunlaşmış olan bu konseptin, Türkiye sağlık sistemi üzerinde yapılan bu çalışmayla, sağlık yönetimi alan yazınında yer alan sağlık okuryazarlığı kavramını desteklemesi ve gelecekteki çalışmalar için kullanılabilecek bir araç olması beklenmektedir.

N2  - Purpose: This study aims to develop a measurement tool and methodology for assessing literacy levels of health insurance, which is a significant component of the health system in terms of financing and access to services.Design/Methodology: A scale development proceses was conducted in the study. An item pool and the measurement structure was shaped based on the characteristics of Turkey's health system financing, studies in the literature, and expert opinions. Data collected in two separate phases included a total of 170 participants. The model was subjected to content validity, construct validity, internal consistency, and test-retest reliability analysis.    Findings: The four-factor structure, which was developed based on the result of the exploratory factor analysis, was found to be reliable and valid in terms of factor loadings and fit indices according to confirmatory factor analysis’ results. It was determined that the social health insurance dimension could be evaluated with the sub-dimensions of “general knowledge level” and “ability to acquire information”, while the private health insurance dimension could be assessed with the sub-dimensions of “ability to use” and “ability to obtain/purchase.'"Limitations: Face-to-face qualitative interviews with individuals from different social and economic groups could improve the process of the development. Also, a community-based item pool development process was not possible as only expert opinions were obtained.Originality/Value: T This study is the first of its kind by offering a tool for measuring health insurance literacy, in a health system where social health insurance is the fundemental of health financing system. The global literature is predominantly focused on private health insurance. With this study conducted on the Turkish health system, it is expected to support the concept of health literacy present in the health management literature and to serve as a tool for future research.
