Materials and Methods: This study was conducted using a descriptive and correlational research design and included 258 participants who were reached by the snowball sampling method in Muğla province. Research data were collected by an online questionnaire method through Google Forms. The participants were administered the Patient Identification Form, the Medication Adherence Self-Efficacy Scale–Short Form, and the Older People’s Quality of Life–Brief. Results: As a result of the analysis, it was determined that there was a negative relationship between medication adherence self-efficacy and quality of life (r=-0.42, p<0.001). Multiple regression analysis revealed that medication adherence self-efficacy was a significant predictor of quality of life (β = -0.38, p < 0.001). In addition, individuals with high medication adherence self-efficacy had lower quality of life scores (Mean Quality of Life Score: 65.2±10.8). Conclusion: In elderly individuals with hypertension, medication compliance self-efficacy significantly affects quality of life. Although high self-efficacy enables the individual to approach treatment more consciously, this may negatively affect quality of life with increased anxiety and awareness of side effects.
