AB  - Amaç: Şizofreni tedavisinde eşlik eden nörolojik ve kardiyak hastalıklar, antipsikotik ilaç seçimini güçleştiren klinik durumlardır. Bu olguda, Parkinson hastalığı ve 1. derece AV blok tanıları bulunan bir hastada, önceki antipsikotiklere bağlı yan etkiler nedeniyle lurasidon tedavisine geçiş süreci, tedavi yanıtı gözlenmiş, sunulmuştur.Olgu: 59 yaşında erkek hasta 20 yıldır şizofreni tanısıyla takip edilmekteydi. Son aylarda artan alınganlık, şüphecilik şikayetleri nedeniyle başvurdu. Eşlik eden Parkinson hastalığı ve 1. derece AV blok tanıları mevcuttu. Daha önce risperidon, paliperidon ve aripiprazol kullanımı sırasında motor semptomlarda kötüleşme ve kardiyak iletimde yavaşlama gözlendi. Bu nedenle tedavide lurasidona geçiş planlandı. Lurasidon 20 mg/gün başlanarak üç hafta içinde 80 mg/gün’e çıkarıldı. Sekizinci haftada Pozitif ve Negatif Sendrom Ölçeği (PANSS) toplam puanı 95’ten 60’a geriledi. Parkinson semptomları ve AV blok bulgularında kötüleşme olmadı.Sonuç: Bu olgu, Parkinson hastalığı ve AV ileti bozukluğu eşlik eden şizofreni hastalarında lurasidonun dikkatli doz titrasyonu ve yakın klinik izlemle etkin ve tolere edilebilir bir seçenek olabileceğini göstermektedir.

N2  - Objective: Comorbid neurological and cardiac disorders pose significant challenges in the pharmacological management of schizophrenia. This case report presents the clinical course and therapeutic outcome of a patient with Parkinson’s disease and first-degree atrioventricular (AV) block who was successfully treated with lurasidone after experiencing adverse effects with other antipsychotics.Case: A 68-year-old male patient with a 20-year history of schizophrenia presented with increased suspiciousness and referential ideas. He had comorbid Parkinson’s disease and first-degree AV block. Previous treatments with risperidone, paliperidone, and aripiprazole had worsened his parkinsonian symptoms and slowed cardiac conduction. Lurasidone was initiated at 20 mg/day and titrated to 80 mg/day within three weeks. After eight weeks, the Positive and Negative Syndrome Scale (PANSS) total score decreased from 95 to 60. Parkinsonian and cardiac findings remained stable throughout treatment.Conclusion: This case suggests that lurasidone may represent an effective and well-tolerated treatment option for schizophrenia in patients with comorbid Parkinson’s disease and AV conduction abnormalities when administered with careful dose titration and close monitoring.
