Here we present the case of a 57-year-old male with a far less common injury, that is, isolated avulsion of the humeral insertion of the subscapularis muscle secondary to minor trauma and without anterior dislocation of the shoulder. The lesion was missed at the initial examination and anteroposterior radiograph and later diagnosed at follow up and confirmed by MRI and CT. Surgical treatment was performed through a deltopectoral approach and transosseous sutures were used for immediate strong repair. The patient had almost full and painless active motion of the left shoulder at four months. Isolated subscapularis tendon tear or avulsion of its insertion in the absence of dislocation is possible even after a minor trauma and it should be actively sought in the presence of anterior shoulder pain and marked limitation of motion.}, number={4}, publisher={Turkish Association of Orthopaedics and Traumatology}
