AB  - In this study we discussed the replantation indications and evaluation methods for major upper and lower extremity amputations. Patients general condition, type of injury, duration of ischemia, level of injury, age, occupations, social and economical status are the main criteria for decision making for replantation of major amputations. Modified Tamai classification suggested by Ipsen seems to be the best method to evaluate long term functional outcomes of major upper extremity replantations. Primary amputations for upper extremity should be last choice provided that there is no life threatening condition. Replantation for lower extremity should only be considered if stability and sensibility would be achieved.
N2  - Bu yazıda majör alt ve üst ekstremite amputasyonlarındaki replantasyon endikasyonlarını ve değerlendirme yön-temlerinı tartıştık. Majör ekstremite amputasyonlarında replantasyon yaparken, hastanın genel durumu, yaralanma tipi. iskemi süresi, amputasyon seviyesi, yaş, sosyo-ekonomik faktörler ve meslek, değerlendirilmesi gereken önemli kriterlerdir. Majör üst ekstremite replantasyonu sonrası uzun dönem fonksiyonel sonuçları değerlendirmede en uygun yöntemin Ipsen’in önerdiği modifiye Tamai sistemi olduğunu düşünüyoruz. Hastanın hayati tehlikesi olmadığı sürece, üst ekstremitede primer amputasyon son seçilecek tedavi yöntemi olmalıdır. Alt ekstremitede ise duyusu olan stabil bir ayak elde edilecekse replantasyon yapılmalıdır.
