Treatment of choice should enable the patients to reach previous functional state. There are a large number of algorithms and scoring systems that help guide the decision making in terms of fracture probability and surgical stabilization. The Mirels scoring system is often used for long bones. When surgical decision is made, it should be planned on indivdual basis. The treatment option depends on the affected bone, the anatomical location, the extent of the disease, and the involment of soft tissue. Since treatment of complications in cancer patients is even more problematic, surgical complications should be minimized. The type of reconstruction chosen should be durable and revision should be avoided throughout the life whenever possible. Radiotherapy and embolization are often used as a local adjuvant after surgery or for palliation. Following oncologic principles in treatment is crucial. Type of fixation or prosthetc reconstruction should be chosen according tolocation of the lesion, grade of bone destruction, and life expectancy.
