HE may have an acute onset, characterized by episodes of cerebral ischemia, seizure, and psychosis, or it may present as an indolent form with depression, cognitive decline, myoclonus, tremors, andfluctuations in level of consciousness. Diagnosis is often difficult due to its clinical presentation. It is diagnosed by the presence of high titers of antithyroid antibodies in serum and/or cerebrospinal fluid; no alteration in the cerebrospinal fluid and/or imaging tests compatible with infectious, vascular, orneoplastic etiology; and a good response to immunosuppressive therapy. Here, we presented a case of 53year-old woman with pre-diagnosis of encehaplitis which is diagnosed as HE.
