N2  - Case Report:A 23-year-old male patient treated with type 1 diabetes mellitus for nine years, was admitted to the emergency department of our hospital with a feeling of blockage in the throat, nasal speech, chest pain, nausea and vomiting. Spontaneous pneumo mediastinum (SPM) was diagnosed by systemic examination and analysis. In this study, the SPM case is presented in cases with diabetic ketoacidosis. SPM is rare and may be associated with diabetic ketoacidosis. Conclusion: Systemic examination is very important in Diabetic ketoacidosis. Feeling of blockage in the throat, chest pain and congestion should be considered in the differential diagnosis of SPM
