N2  - Appendix appearing in an inguinal hernia sac is know as ‘Amyand’s hernia. It is rare to find a perforated appendix within an inguinal hernia. We report an elderly female who was admitted to our emergency department with a 5-day history of painful right groin mass. Signs and symptoms of acute appendicitis are overshadowed by clinical findings related to incarcerated inguinal hernia and thus preoperative diagnosis of Amyand Hernia may be difficult. İn this case the hernia was thought to be strangulated and the patient underwent surgery. The appendix was totally in the hernia sac with it’s mesentery. Appendectomy was performed and direct inguinal hernia was repaired with prolene mesh. However it is not usually recognized before exploration, clinicians are encouraged to be aware of this unusual entity. Otherwise acute appendicitis within an Amyand hernia can be life threatening.
