N2  - Objective: In this study, we aimed to compare the efficacy and side effects of morphine and tramadol in patient controlled analgesia after lower extremity arthroplasty. Material and Methods: Records of the patients who under went elective lower extremity arthroplasty between January 2012December were  retrospectively evaluated. Patients' age, gender, American society of anesthesiologists classification, type of the operation, analgesic and anesthetic drugs used in intraoperative period, total analgesic drug consumption in patient controlled analgesia, visuel analog scale in postoperative 1., 2., 6, 12, 24. hours and postoperative side effects were analyzed and presented. Results: A total of 70 patients were enrolled in this study. Patients' visuel analogue scale pais cores in postoperative 1. and 2. hours were significantly higher in tramadol group than morphine group (respectively p=0.008, p=0.008). Mean of 24 hours total drug consumption was  362.96±60.52 mg in tramadol group and 27.39±7.00 mg in morphine group. Nausea and  vomiting incidence, additional analgesic requirement and the number of patients whose bolus dose was increased were significantly higher in tramadol group (respectively p=0.048, p=0.036, p=0.018). Conclusion: It can be concluded that Morphine and Tramadol in patient controlled analgesia produce equivalent pain scores but morphine offers advantage providing less additional analgesic consumption and lower incidence of nause and vomiting after arthroplasty.
