For coronary arter perforations, Ellis classification is used. The rate of death in patients with Ellis class 3 perforation has been about 44%. A 47-years old man with a history of coronary artery by-pass was hospitalised with a diagnosis of unstable angina. The Ellis class 3 perforation following routine stenting of saphenous vein graft stenosis was successfully treated by a polytetrafluoroethylene-covered graft stent. The coronary perforation following percutaneous coronary interventions should not be forgotten.
