Low-flow / low-gradient (LF/LG) aortic stenosis and reduced left ventricular ejection fraction (EF) occurs only in 5% of patients with aortic stenosis. They have a poor prognosis with conservative therapy but a high operative mortality when treated surgically aortic valve replace-ment (AVR). The improvement in left ventricular EF after surgical AVR varies extensively from one patient to another, and EF often re-mains abnormal. Moreover, AVR is associated with markedly increased operative mortality in patients with severe AS and reduced EF. Recently, transcatheter aortic valve implantation (TAVI) has emerged as an alternative to surgical AVR for patients with high operative risk and severe comorbidities.
