AB  - Fibromiyalji sendromu yaygın vücut ağrısı ve belirlianatomik bölgelerde hassas noktaların varlığı ile karakterize, etiyoloji netolarak bilinmeyen, iş gücü kaybı ve yaşam kalitesinde bozulmaya yol açan kronikbir kas-iskelet sistemi hastalığıdır. Fibromiyaljisendromunda en sık görülen ve karakteristik semptomlar yaygın ağrı, sabahkatılığı, yorgun uyanma ve uyku bozukluğudur. Aile öyküsü olanlardafibromiyaljinin daha fazla görülmesi genetik faktörlerin rol aldığınıdüşündürmektedir. Fibromiyaljili hastalarınağrılı uyaranlara daha duyarlı olduğu gösterilmiştir. Fibromiyalji sendromununetiyopatogenezi üzerine uzun yıllardır birçok çalışma yapılmakta olupmultifaktöryel mekanizmalar olduğu görüşü ağırlık kazanmaktadır. Bu faktörlergenetik nedenler, immunolojik mekanizmalar ve santral ve periferik teorilerolarak alt başlıklarda toplanmaktadır. Fibromiyaljinin sıklıkla santralsensitivite sendromları ile beraber olması, glutamat ve P maddesi gibi ağrınörotransmitterlerinin artmış olması, noradrenalin, serotonin ve dopamin gibiantinosiseptif nörotransmitter düzeylerinin azalmış olması, ektopikdeşarjlar/periefrik sensitizasyon/ Aβ liflerinin reorganizasyonu vehiperaljezi ve allodinin varlığı fibromiyaljinin nöropatik ağrı sendromun birparçası olabileceğini düşündürmektedir.Son yıllardayapılan çalışmalar santral mekanizmaların etkinliğini desteklemektedir Nöropatik ağrı ve fibromiyalji sendromu ilişkisi literatürde yapılan az sayıda çalışmadagösterilmiştir. Son yıllardaki çalışmalarda fibromiyalji sendromuile küçük lif polinöropatisi sıklığının saptanması ile periferik sistemindeolaya katkı sağladığı görüşü gündeme gelmiştir Buyazıda fibromiyalji sendromu ve nöropatik ağrı son gelişmeler ışığında tekrargözden geçirilecektir.
