AB  - Boyunfleksiyonu ile ekstremitelere ve gövdeye yayılan elektriklenme hissi Lhermittebulgusu olarak tanımlanmaktadır ve spinal kordun servikal bölgede etkilenmiş olabileceğinidüşündürmektedir. Altmış dokuz yaşında erkek hastaya servikal spinal stenoz vemyelopati nedeniyle yapılan C3-4-5 unilateral hemilaminektomibilateral dekompresyon sonrası 12. saatte hastanı kollara ve bacaklara yayılanelektriklenme hissi tariflemesi üzerine yapılan servikal manyetik rezonansgörüntülemesinde operasyon bölgesinde hematom görülmesi üzerine tekraroperasyona alındı. Hematom boşaltılmasını takiben bu bulgu kayboldu. Lhermittebulgusunun görüldüğü klinik durumlar çok çeşitli olabilmektedir. Servikalkordun posterior kısımlarındaki demiyelinizasyon sonucu ortaya çıktığıdüşünülür. Sensitif bir bulgu olmamasına rağmen servikal kompresif myelopatiiçin spesifitesi yüksektir. Operasyon sonrası Lhermitte bulgusunun görülmesikord kompresyonuna işaret edebileceğinden dikkatli olunmalı ve ileri tetkiklerdüşünülmelidir.

N2  - Lhermitte’ssign describes an electric shock-like sensation that radiates down the trunkand limbs with flexion of the neck. This symptom may show injury of spinal cordat cervical levels. A 69-years-old male had C3-4-5 cervical spinalstenosis and myelopathy. The patient described an electric shock-like sensationwhich radiated to limbs at postoperative 12th hour after C3-4-5 leftsided unilateral hemilaminectomy and bilateral decompression. There was anepidural hematoma that compressed spinal cord at the level of hemilaminectomyin magnetic resonance imaging. We performed an immediate surgery and evacuatedthe hematoma. Lhermitte’s sign dissapeared after surgery. Lhermitte’s signcould be seen in various conditions and the pathophysiology is described as thedemyelination of dorsal column of the spinal cord, at the cervical level. Ithas poor sensitivity but good specificity in cervical compressive myelopathy.Postoperativenew onset Lhermitte’s sign can indicate cord compression. The patient must beevaluated with further examinations.
