AB  - Tetanos, Clostridiumtetani’nin ürettiği ekzotoksinlerin neden olduğu, nadir ve ölümcülseyredebilen bir hastalıktır. Çocukluk çağının hiçbir aşısı yapılmamış olan vemülteci olarak ükemize gelen yedi yaş erkek hasta tüm vücutta kasılma ve ağrışikâyeti ile başvurdu. Alınan anamnezde bir hafta önce düşme sonrası ayağınapaslı çivi batma öyküsü olduğu ve sonrasında herhangi bir sağlık kuruluşunabaşvurmadığı öğrenildi. Hastaya tetanos tanısı konuldu ve çocuk yoğun bakımünitesinde izleme alındı. Hastaya tetanos aşısı ve tetanos immünglobulinyapıldı. Penisilin tedavisi başlandı. Yara yeri debride edildi. Takibindekasılmaları artan ve solunum sıkıntısı gelişen hasta entübe edildi. Sonrasındaarrest olan hastaya kardiyopulmoner resusitasyon uygulandı. Hastanın izlemindeakut böbrek yetmezliği ve dissemine intravaskülerkoagülasyon gelişmesi üzerine çoklu organ yetmezliği düşünülen hasta takibinde exitusoldu. Sonuç olarak, görülme sıklığındaki artış nedeni ile özellikle yaralanmalarsonrası santral sinir sistemi bulguları ile başvuran hastalarda aşı öyküsümutlaka sorgulanmalı ve ayırıcı tanıda tetanos düşünülmelidir.

N2  - Tetanus is a rare and lethal diseasethat results from exposure to exotoxins produced by Clostridium tetani. A seven year-old-male refugee who had not beenvaccinated in early childhood was admitted to the hospital with generalized musclespasm and pain. The anamnesis taken from the patient revealed that he hadsuffered from nail injury after falling down a week ago and didn’t take anymedical care. He was diagnosed with tetanus and admitted to the intensive careunit. He was given tetanus vaccine and tetanus immunoglobulins. Penicilinetreatment was started. The wound was surgically debrided. The patient was intubated due to increased musclespasms and respiratory distress. After that the patient who went into cardiacarrest was necessitated cardiopulmonary resuscitation. The patient whodeveloped acute renal failure and disseminated intravascular coagulation was thoughtto have multiple organ failure. And the patient later died during the follow up. As a result; due to the increased incidence,the patients who have presented with symptoms of central nervous system,especially after injuries, must be inquired about the vaccination history andtetanus must be taken into consideration in the differential diagnosis.
