Kalp Yetersizliğine Özgü Sağlık Okuryazarlığı Ölçeğinin Türkçe Uyarlaması: Geçerlilik, Ölçüm Değişmezliği ve Güvenilirlik Çalışması

Abstract

Amaç: Bu çalışmanın amacı kalp yetersizliğine özgü sağlık okuryazarlığı ölçeğini Türkçeye uyarlamaktır. Araçlar ve Yöntem: Metodolojik bir çalışmadır: Bu metodolojik çalışma Ocak-Mayıs 2024 tarihleri arasında Türkiye'nin doğusundaki bir hastanenin kardiyoloji polikliniğine başvuran 748 kalp yetersizliği hastası ile yürütülmüştür. Veri toplamak için bir kişisel bilgi formu ve kalp yetersizliğine özgü bir sağlık okuryazarlığı ölçeği kullanılmıştır. Ölçek geçerliliği için; açıklayıcı faktör analizi, doğrulayıcı faktör analizi, ölçüm değişmezliği analizi, yakınsak ve ıraksak geçerlilik analizleri yapılmıştır. Güvenirlik için ise cronbach alpha ve Mc Donald's omega iç tutarlılık katsayısı, düzeltilmiş madde toplam korelasyonu ve sınıf içi korelasyon katsayısı analizleri yapılmıştır. Bulgular: Çalışmadaki iki veri setinin kişisel özellikleri arasında anlamlı bir fark yoktur (p˃005). Kapsam geçerlilik indeksi 0.93'tür. Açıklayıcı faktör analizi, faktör yükleri 0.74-0.87 arasında değişen üç faktörlü bir yapı ortaya koymuştur. Doğrulayıcı faktör analizinde, üç faktörlü model için uyum iyiliği indekslerinden biri kabul edilebilir uyum, diğer altısı ise mükemmel uyum göstermiştir. Ölçeğin cinsiyetler arası ölçüm değişmezliğinin tatmin edici düzeyde olduğu ve modeller arası farklılık kriterlerini (yapısal, metrik, skaler ve katı) karşıladığı belirlenmiştir. Ölçeğin Cronbach alfa katsayısı 0.73, Mc Donald's omega katsayısı ise 0.74’tür. Sonuç: Kalp yetersizliğine özgü sağlık okuryazarlığı ölçeği Türk toplumunda yüksek düzeyde geçerli ve güvenilir bir ölçüm aracı olarak bulunmuştur.

Turkish Adaptation of Heart Failure Specific Health Literacy Scale: Validity, Measurement Invariance and Reliability Study

Abstract

Purpose: The aim of this study was to adapt a heart failure-specific health literacy scale into Turkish. Materials and Methods: This methodological study was conducted between January and May 2024 with 748 heart failure patients who visited the cardiology outpatient clinic of a hospital in eastern Turkey. Data were collected using a personal information form and a heart failure-specific health literacy scale. To assess the scale's validity, exploratory factor analysis, confirmatory factor analysis, measurement invariance analysis, and convergent and divergent validity analysis were performed. For reliability, Cronbach's alpha, McDonald's omega internal consistency coefficient, corrected item-total correlation, and intraclass correlation coefficient analyses were conducted. Results: There was no significant difference between the personal characteristics of the two data sets in the study (p>0.05). The content validity index was 0.93. Exploratory factor analysis revealed a three-factor structure with factor loadings ranging from 0.74 to 0.87. In confirmatory factor analysis, one of the goodness-of-fit indices for the three-factor model showed an acceptable fit, while the other six indices indicated excellent fit. The measurement invariance of the scale between genders was found to be satisfactory, meeting the criteria for differences between models (configural, metric, scalar, and strict). The Cronbach's alpha coefficient of the scale was 0.73, and McDonald's omega coefficient was 0.74. Conclusion: The heart failure-specific health literacy scale was found to be a higly valid and reliable measurement tool for the Turkish population.
