                                                                                                <abstract><p>Giriş:  Kronik hepatit B (KHB) hastalarında evreleme amacıyla karaciğer biyopsisi hâlâ altın standarttır. Ancak kullanılan teknikler ve uygulayıcı branş, doku kalitesi, güvenlik ve sağlık kaynaklarının kullanımını etkileyebilir. Gastroenteroloji (GE) ve girişimsel radyoloji (GR) bölümleri bu işlemi farklı yöntemlerle gerçekleştirmektedir.Amaç: 2024 BSG güncel kılavuzları ve Türkiye geri ödeme sistemi çerçevesinde, GE ve GR ekipleri tarafından yapılan biyopsilerin örnek kalitesi, güvenlik ve maliyet açısından karşılaştırılması.Yöntem: Bu retrospektif, tek merkezli çalışmada, Şubat 2022 – Kasım 2023 tarihleri arasında KHB tanılı 81 hasta değerlendirildi. GE ekibi interkostal yolla 16G manuel iğne, GR ekibi ise subkostal yolla 18G otomatik tru-cut iğne kullandı. Demografik, laboratuvar, histopatolojik ve takip verileri analiz edildi. Bulgular: GE ekibi tarafından elde edilen biyopsilerde örnek uzunluğu ve portal trakt sayısı anlamlı şekilde daha yüksekti (p=0,006). Her iki grupta tanısal yeterlilik %100’dü.                             <p>Background:Liver biopsy remains the gold standard for staging in patients with chronic hepatitis B (CHB). However, the technique used and the operator’s specialty can influence tissue quality, safety, and healthcare resource utilization. Both gastroenterology (GE) and interventional radiology (IR) departments perform liver biopsies using different approaches.Objective:To compare biopsy specimen quality, safety, and cost between GE and IR teams within the framework of the 2024 British Society of Gastroenterology (BSG) guidelines and the current reimbursement system in Türkiye.Methods:This retrospective single-center study evaluated 81 patients with CHB who underwent liver biopsy between February 2022 and November 2023. The GE team used a 16G manual needle via the intercostal route, while the IR team used an 18G automatic tru-cut needle via the subcostal route. Demographic, laboratory, histopathological, and follow-up data were analyzed.Results:Biopsies performed by the GE team yielded significantly longer samples and a higher number of portal tracts (p=0.006). Both groups achieved 100% diagnostic adequacy. ALT, INR, and HBV DNA levels were higher in the GE group.
