KOAH Hastaları Hastaneden Çıkmıyor mu?

Öz

Amaç: ‘KOAH (Kronik Obstrüktif Akciğer Hastalığı) hastaları hastaneye çok sık yatıyor ve yatınca da kolay kolay taburcu olmuyor.” ön yargısı sıklıkla dile getirilmektedir.  Çalışmamızda bu ön yargının doğru olup olmadığını araştırmayı amaçladık.

Materyal ve Metod: Çorum Göğüs Hastalıkları Hastanesi’nde 1 Ocak 2009-31 Aralık 2010 tarihleri arasında KOAH tanısı ile yatan hastaların yatış sayıları ve süreleri ile ilgili veriler hastane bilgi ve yönetim sisteminden (HBYS) alınarak retrospektif olarak değerlendirilmiştir. 

Bulgular: Çorum Göğüs Hastalıkları Hastanesi’nde 2009 ve 2010 yıllarında toplam yatış sayısı sırasıyla 5.148 ve 5.420 olup toplam 10.568’dir. Tüm yatış nedenleri arasında KOAH’a bağlı yatış sayısı 2009 yılında 2.046 (% 39,7), 2010 yılında 2.147 (%39,6), toplamda ise 4.193 (%39,6) olup ilk sırada yer almaktadır. 2009 yılında 1.458 KOAH’lı hasta toplamda 2.046 kez hastaneye yatırılmıştır. Hastaların %76,8’i sadece 1 kez, %14,5’i 2 kez yatırılmıştır.  Aynı hastanın 3 veya daha fazla olan yatışları sık hospitalizasyon olarak değerlendirildiğinde 2009 yılında hastaların % 8,5’i bu grupta yer almıştır. 2010 yılında 1.523 KOAH’lı hasta toplamda 2.147 kez yatırılmıştır. Hastaların %76,6’sı sadece 1 kez, %14,3’ü 2 kez yatırılmıştır. 2010 yılında hastaların % 8,8’i sık hospitalizasyon gurubunda yer almıştır. 2009 ile 2010 yılları karşılaştırıldığında oranlar benzer bulunmuştur. KOAH hastalarının ortalama yatış süreleri 2009 yılında 9,82 gün/hasta iken 2010 yılında 10,6 gün/hasta’dır. 

Sonuç: KOAH hastalarında mükerrer yatış oranları düşük tespit edilmiş olup, yatış süreleri beklenildiğinden düşük çıkmıştır.

Are COPD patients not discharged from hospital?

Öz

Objective: “COPD (Chronic Obstructive Pulmonary Disease) patients are often hospitalized and not easily discharged” prejudice is often expressed. We aimed to investigate whether this bias is correct or not in our study.  

Methods: Data about duration and number of hospitalizations of COPD patients hospitalized between January 1, 2009 and December 31, 2010 in Corum Chest Diseases Hospital was obtained from hospital information management system (HIMS) and evaluated retrospectively.

Results: Number of hospitalizations in Çorum Chest Diseases Hospital was 5148 and 5420 respectively in 2009 and 2010 (Total 10568). Number of hospitalization due to COPD was 2046 (%39.7) in 2009, 2147 (%39,6) in 2010 and 4193 (%39,6) totally; ranking first. In 2009, 1458 COPD patients were hospitalized 2,046 times. 76.8% of the patients were hospitalized only once, 14.5% of them twice. Hospitalization of three or more of the same patient, which is considered as frequent hospitalization; 8.5% of patients in 2009 were included in this group. In 2010, a total of 1,523 COPD patients were hospitalized 2,147 times. 76.6% of patients were hospitalized only once, 14.3% twice. In 2010, 8.8% of patients were included in the group of frequent hospitalization. In 2010 compared with 2009 rates were similar. The average length of stay of patients with COPD in 2009 is 9.82 days / patient and  10.6 days / patient in 2010.

Conclusion: Low repeating hospitalization rates in patients with COPD has been identified, duration of hospitalization was lower than expected
