Frontal Osteoma ile Frontal Sinüs Büyüklüğü Arasındaki İlişki ve Bunun Cerrahi Yöntem Seçimine Etkisi

Abstract

Objectives: The size and function of the sinuses are influenced by many factors. For this purpose, we planned to investigate the relationship between frontal osteoma dimensions, localization, and presence of osteoma and frontal sinus size and the rate of surgical intervention. Materials and Methods: After approval of the ethics committee, the archive images of 146 patients with frontal osteoma detected by 27616 paranasal sinus CTs taken between 2014-2019 were retrospectively reviewed. The distance between the most medial and lateral points of the frontal sinus was measured by coronal and horizontal measurements;the distance between the most cephalic and caudal points and the horizontal line drawn to the upper boundary of the orbital distance was measured with reference to the most caudal distance. We investigated whether there was a significant relationship between osteoma size and frontal sinus size and going to surgery. Results: There was no significant difference between the right and left frontal sinus dimensions of the patient and control groups. 86.3% of the osteoma group was found to be stage 1. In the whole osteoma group only 8.2% had undergone surgical intervention. The measured osteoma width,height and height values of male patients were higher than female patients. There was no difference between the percentages of osteoma, osteoma size and right and left frontal sinus sizes according to osteoma side of the patients. While a positive correlation was observed between the sinus widths of the patients with osteoma and sinus widths without osteoma, sinus widths with osteoma and sinus length without osteoma (r=0.400, p<0.001) and heights. Conclusion: The presence of an osteoma does not negatively affect sinus dimensions on the involved side and the opposite side. As long as the osteoma is not enlarged, it may be asymptomatic for a long time. The choice of surgical approach depends on the location and size of the osteoma.

Abstract

Amaç: Sinüslerin boyutları ve işlevleri birçok faktör tarafından etkilenmektedir. Bu amaçla frontal osteoma sıklığı, boyutları, lokalizasyonu, osteoma varlığının frontal sinüs büyüklüğü ile ilişkisini ve cerrahi müdahale oranını araştırmayı planladık. Gereç ve Yöntem: Etik kurul onayı alındıktan sonra, 2014-2019 yılları arasında çekilen 27616 paranazal sinüs BT sonucunda frontal osteom saptanan 146 hastanın arşiv görüntüleri retrospektif olarak incelendi. Koronal ve horizontal planda ölçümler yapılarak frontal sinüs en medial ve en lateral noktaları arasındaki uzaklık; en sefalik ve en kaudal noktaları arasındaki uzaklık ve orbital üst sınırına çizilen horizontal çizgi referans alınarak en kaudale uzaklık ölçüldü. Hastaların muayene kayıtlarından geliş şikayetleri ve cerrahi müdahale oranı tespit edildi. Osteom boyutu ve frontal sinüs büyüklüğü ile cerrahiye gidiş arasında anlamlı bir ilişki olup olmadığı araştırıldı. Bulgular: Hasta ve kontrol grubunun sağ ve sol frontal sinüs boyutları arasında istatistiksel olarak anlamlı bir fark yoktu (p>0,05). Osteomlu grubun %86,3’ü evre 1 olarak bulundu. Tüm osteomlu grupta sadece %8,2’sine cerrahi müdahale uygulanmıştı. Erkek hastaların ölçülen osteom en, boy ve yükseklik değerleri kadın hastalara göre istatistiksel olarak daha yüksek bulundu (p<0,05). Hastaların osteom tarafına göre cinsiyet yüzdeleri, osteom boyutları ve sağ ve sol frontal sinüs boyutları karşılaştırıldığında istatistiksel olarak anlamlı bir fark olmadığı görüldü (p>0,05). Hastaların osteomlu sinüs enleri ile osteomsuz sinüs enleri arasında istatistiksel olarak anlamlı orta derecede pozitif bir korelasyon gözlenirken (r=0,532, p<0,001), osteomlu sinüs enleri ile osteomsuz sinüs boy (r=0,400, p<0,001) ve yükseklikleri (r=0,398, p<0,001) arasında pozitif ve zayıf bir korelasyon olduğu görüldü. Sonuç: Osteom olması tutulum olan tarafta ve karşı tarafta sinüs boyutlarını negatif olarak etkilememektedir. Osteom büyümediği müddetçe uzun süre asemptomatik seyir gösterebilir. Cerrahi yaklaşım seçiminde osteomun yerleşim yeri ve büyüklüğüne göre karar verilir.
