Psoriazis Hastalarında Siklosporin Kullan m n n Etkinlik ve Güvenilirliği; Retrospektif Çal Ĺma

Abstract

şiddetli seyir gösteren psoriazis vakalarında siklosporin, hızlı klinik yanıt elde edilmesi için kullanılan bir ajandır. Son senelerde, konvansiyonel tedavi ajanlarına ilişkin gözlemsel çalışmalar az sayıda bulunmaktadır. Bu nedenler, bu çallşmada, psoriazis tanısı ile 6 ay ve daha uzun süreli siklosporin kullanmanın etkinliğinin ve güvenliğinin araştırılması planlanmıştır. Gereç-Yöntem: Ocak 2005 ile şubat 2013 tarihleri arasında Ankara Üniversitesi Tıp Fakültesi Deri ve Zührevi Hastalıklar Anabilimdalı ’nda psoriazis tanısı ile izlenen, 6 ay ve daha uzun süreli siklosporin kullanan hastalar çalışmaya dahil edilmiştir. Klinik etkinlikte PASI skorundaki ölçüt alınmıştır. Gelinen yan etkiler, siklosporin tedavisinin güvenliliğinin değerlendirilmesi amacı ile kaydedilmiştir. Bulgular: Plak veya eritrodermik psoriazis tanısı almış olan 20 hasta çalışmaya dâhil edilmiştir. Hastaların tedavi öncesi PASI değerleri ortalama 13,3 (±4,8 SD) iken, 4. hafta PASI değeri ortalama 4,9 (±2,5 SD), 8. hafta PASI değeri 2,8 (±1,9 SD) ve 12. hafta PASI değeri 1,3 (±1,5 SD) olarak hesaplanmıştır. Siklosporin sonras PASI deþerlerinde anlaml düĹüĹ elde edilmiĹtir. Tüm hastalar n böbrek fonksiyonlar ve tansiyon değerleri incelenmiştir. Hastaların hiçbirinde serum kreatinin değerlerinde %30'dan fazla yükselme görülmemiştir. 2 hastada antihipertansiflere yanıtl l ml hipertansiyon gelişmiştir. Sonuç: Bulgular eşliğinde, psoriazis hastalarında siklosporinin hızlı yanıt elde edebilme açısından etkin bir ajan olduþu söylenebilir. Çal Ĺmaya dahil edilen hasta grubunda ciddi yan etki gelişimi gözlenmemiş olma-sına karşın, hastalar, yan etki gelişimi açısından yakin takip altında tutulmalıdır.

Abstract

Background: Cyclosporine is used in recalcitrant patients with psoriasis to obtain rapid healing. In recent years, only few studies have been published about conventional theurapetic agents such as cyclosporine. Herein, we aim to investigate efficacy and safety of cyclosporine therapy for a duration of 6 months or more, retrospectively. Materials and Methods: Psoriasis vulgaris or erithrodermic psoriasis patients routinely examined and used cyclosporine for 6 months or more between January 2005 and February 2013 at Ankara University Faculty of Medicine, Department of Dermatology and Venereologyand were included in this study. PASI score was used to measure clinical efficacy and advers effects were used to measure safety of cyclosporine. Results: Twenty patients with either plaque or erythrodermic psoriasis were included in the study . The mean pre- treatment PASI scores of patients were 13.3 ( ± 4.8 SD ), while 4. weeks PASI scores were 4.9 ( ± 2.5 SD) , 8 weeks PASI scores were 2.8 ( ± 1.9 SD) and 12 weeks PASI scores were 1.3 ( ± 1.5 SD. Significant decrease in PASI score was observed after initiation of cyclosporine therapy. All patients were examined by blok creatinine levels and blood pressure values None of the patients had elevated creatinine levels more than 30% of normal values. Two patients developed moderate hypertension which was responsive to antihypertensive treatment. Conclusion: Cyclosporine is an effective theurapeutic agent which provides rapid healing in patients with psoriasis vulgaris and erythrodermic psoriasis with acceptable relapse rate. No serious adverse effects was observed in patients included in this study, however patients on cyclosporine therapy should be monitored closely in terms of side effects.
