Prostat Kanserli Hastalarda Bilateral Orkiektomi ile Bilateral Orkioktemi - CPA Tedavisinin Karşılaştırılması

D evresindeki 32 prostat kanserli hastadan 16'sına bilateral orkiektomi kalan 16'sına da bilateral orkiektomi ile birlikte anti-androjen olan cypteron acetate (CPA) (100mg/gün) vererek anti androjen tedavinin klinik etkinliklerini araştırdık. Orkiektomi grubunda ilk yılda %67 CPA grubunda ise %75 başarılı yanıt alındı. İkinci yılda ise grupta %58 diğer grupta %43 oranında progresyon görüldü. Literatür uyumlu olarak CPA ile kombine anti-androjen tedavinin cerrahi kastrasyona üstünlüğünü gözlemedik. Ancak CPA grubunda sıcak basması gibi komplikasyonların anlamlı olarak azlığı tedaviye bu yönde üstünlük kazandırıyordu.

We investigated the clinical efficacy of anti-androgen therapy by giving 16 patients with bilateral orchiectomy and the remaining 16 patients with bilateral orchiectomy and cypteron acetate (CPA) (100 mg/day), an antiandrogen. In the first year, 67% of the patients in the orchiectomy group and 75% in the CPA group were successful. In the second year, 58% of the patients in the group and 43% in the other group were seen to progress. Consistent with the literature, we did not observe the superiority of combined anti-androgen therapy with CPA over surgical castration. However, the significant decrease in complications such as hot flashes in the CPA group gave the treatment an advantage in this respect.
