Türkiye'de Okul Çağı Öncesi Çocuklarda Laktaz Yetmezliği

Abstract

3 -6 yaş grubunda olan 89 çocukta laktaz yetmezliği H. nefes testi ile belirlendi. Laktoz 1 g/Kg olarak verildi. 89 olgunun 61'i (96 68,5) laktoz normal, 28'i (& 31,4) laktoz yetmezlikli olarak saptandı. Laktoz yetmezliği prevalansı 3 yaş grubunda “0 13,79, 4 yaş grubunda, 96 30,5 yaş grubunda “ 28,57 6 yaş grubunda ise “o 63,16 idi. Aynı yaş grubundaki çocuklarda laktoz normal ve yetmezliklilerin ağırlıkları arasında istatistik farklılık yoktu. Laktaz normal (24 olgu) ve laktaz yetmezlikli (12 olgu) olgularda Duphalac (20g lactulose) ile yapılan testlerde, bütün olgularda açlıkH, değerlerine göre 20 ppm'den fazla artış husule geldi. Laktaz normal grupta haftalık süt tüketimi, yetmezlikli gruptan daha fazla bulundu (p< 0.05). 89 olgunun 84'ünde süte tahammülsüzlük hikayesi ve test esnasında entolerans mevcut değildi. 89 olgunun yalnız birinde (laktaz normal olgu) gaitada parazit yumurtası tesbit edildi. Bu çalışma Türkiye'de primer laktaz yetmezliğinin 3 yaşında görülmeğe başladığını ve yaş ilerledikçe arttığını göstermektedir

Lactase Insuffıcıency In Preschool Chıldren In Türkiye

Abstract

Prevalence of lactase deficiency in kindergarten children aged 3-6 years was investigated by using breath hydrogen test, Mm the test,a rise in breath hydrogen concentration of > 20 ppm, after the oral lactose load, was considered to be evidence of lactase deficiency. After a lactose dose of 1 g/Kg, 69 children (68,5 © )was İound lactase normal,whereas 28 children (31,4 ©) was observed to be lactase deficient. The incidence of lactase deficiency was 13,79 96 in 3 year, 30 © in 4 year, 28,57 96 in 5 year and 63,16 7 in 6 year old children. The difference in the weights of the children in the same age group was not statisticalIy significant.In 36 cases, lactase normal (24 cases) and lactase deficient (12 cases), the tests carried out after an overnight fast by using Duphalac (20 g lactulose) resulted in more than 20 ppm increase in breath H. excretion. None of these subjects were low hydrogen producers. The weekly milk consumption in lactase normal group was observed to be higher than the lactase dificient group. In 84 of the 89 cases, the lactose intolerance during tests and the history of milk intolerance were not observed. The parasite was found in feces of only one case (lactase normal) out of 89 subjecis. Thus, in this study, the incidence of primary lactase deficiency in Turkey was found to be observed at the age of three and gradually increase by age.
