Akciğer Kist Hidatik Olgularında Hücresel Ve Humoral İmmünitenin Değerlendirilmesi

Abstract

Akciğer kist hidatiği olgularında hücresel ve humoral immünite düzeyini araştırmak gayesi ile 31 hasta üzerinde cilt testi, T lenfosit sayısı, serum immünglobulin, kompleman değerleri ve perikistik doku ile cilt dokusunda immünfloresan tetkik yapılmıştır, T lenfosit sayısı, olguların büyük bir kısmında normal değerlerin altında saptanmıştır. P< 0.001 Cilt testi cevabı ile karşılaştırılmasında bir korelasyon kurulamamışr. Serum immünglobulinlerinden IgA değerlerinde düşme, P< 0.001 ile anlamlı idi. IgM düzeyleri normal değerlerin üstünde olup P< 0.001 ile önemli derecede anlamlı olduğu gözlendi. IgG düzeyleri ise olguların yarısında yüksek değerlerde idi. P< 0.01 Serum kompleman düzeyleri kontrol grubu ile karşılaştırıldığında önemli bir fark kaydedilmemiştir. 13 akciğer kist hidatik olgusunun 9'unda perikistik dokuda direk immünfloresan metod ile elastik liflerde, bronşiol çevresinde IgG ve IgM birikimi, 4 olguda yalnız IgM birikimi izlendi. Cit dokusunda immünfloresan çalışma 4 olguda yapılabilmiştir. Olgu sayısı az olduğundan değerlendirme güç oldu. 4 olguda da dermada elastik, kollajen lifler de ve arteriol çevrseinde 1gG, 1 olguda 1gG ve IgM birikimi saptanmıştır. Perikistik dokuda antijen antikor kompleksi oluşumu antijenin kist duvarına sizması ile izah edilebilir, ancak olguların hepsinde görülmemesi antijenik farklılık, konakçının immün cevabı ile yanıtlanabilir düşüncesindeyiz.

Evaluatıon Of Cellular And Humoral Immunıty In Lung Cyst Hydatıc Cases

Abstract

In order to investigate the level of cellular and humoral immunity in cases of pulmonary hydatid cyst, skin test, T lymphocyte count, serum immunoglobulin, complement values ​​and pericystic tissue and skin tissue immunofluorescence examination were performed on 31 patients. T lymphocyte count was found to be below normal values ​​in most of the cases. P< 0.001 No correlation could be established in the comparison with skin test response. Decrease in IgA values ​​from serum immunoglobulins was significant with P< 0.001. IgM levels were above normal values ​​and were observed to be significantly significant with P< 0.001. IgG levels were at high values ​​in half of the cases. P< 0.01 No significant difference was recorded when serum complement levels were compared with the control group. In 9 of 13 pulmonary hydatid cyst cases, IgG and IgM accumulation was observed in elastic fibers and around bronchioles by direct immunofluorescence method in pericystic tissue, and only IgM accumulation was observed in 4 cases. Immunofluorescence study in skin tissue was performed in 4 cases. Since the number of cases was low, evaluation was difficult. In 4 cases, 1gG accumulation was detected in elastic fibers, collagen fibers and around arterioles, and in 1 case, 1gG and IgM accumulation was detected. Antigen antibody complex formation in pericystic tissue can be explained by the leakage of antigen into the cyst wall, but we think that the fact that it is not seen in all cases can be answered by antigenic differences and the immune response of the host.
