The Relationship with Postpartum Depression of Smoking Use in Women in the Postpartum Period: A Case-Control Study

Abstract

This study was conducted to determine the relationship between smoking and postpartum depression in women in postpartum period. The population of the study, which was conducted as a case-control study, consisted of 1237 women in the postpartum period who were registered in seven Family Health Centers located in a district in a city center, and 294 women in the sample. In study, among women who applied to family health centers for postpartum follow-ups between 25.06.2022 and 25.12.2022, completed their 2nd month after birth, had a baby of 2-12 months, spoke and understood Turkish, after the Edinburgh Postpartum Depression Scale was applied, simple randomization according to depression status 147 women with postpartum depression (case group) and 147 women without postpartum depression (control group) were selected by sampling method. Data of the study were collected by the researcher by interviewing women face to face in a suitable interview room in family health center and using the Personal Information Form, Fagerström Nicotine Dependence Test and Edinburgh Postpartum Depression Scale. It was determined that 51.6% of women in the control group and 51.9% of women in the case group smoked, 16.3% of women in the control group and 4.6% of women in the case group had very high addiction levels. A significant, positive and weak correlation was found between the Edinburgh Postpartum Depression Scale total score average and the Fagerstrom Nicotine Dependence Test total score mean of the case group (p<0.05). Logistic regression analysis revealed that women with very high levels of nicotine dependence were 7.34 times more likely to have postpartum depression than women with very low levels of nicotine dependence. It was revealed that there was no relationship between smoking status women in the case and control groups and postpartum depression, however, as the depression levels of women in the case group increased, their nicotine addiction levels increased.

Doğum Sonrası Dönemde Kadınlardaki Sigara Kullanımının Postpartum Depresyon ile İlişkisi: Vaka-Kontrol Çalışması

Abstract

Bu çalışma, doğum sonrası dönemde kadınlardaki sigara kullanımının postpartum depresyon ile ilişkisini belirlemek amacıyla yapılmıştır. Vaka-kontrol çalışması olarak yürütülen araştırmanın evrenini, bir il merkezindeki bir ilçede yer alan yedi Aile Sağlığı Merkezine kayıtlı olan doğum sonrası dönemdeki 1237 kadın, örneklemini ise 294 kadın oluşturmuştur. Araştırmada 25.06.2022-25.12.2022 tarihleri arasında doğum sonrası izlemleri için aile sağlığı merkezlerine başvuran, doğum sonrası 2. ayını doldurmuş olup 2-12 aylık bebeği olan, Türkçe konuşan ve anlayan kadınlar arasından Edinburg Postpartum Depresyon Ölçeği uygulandıktan sonra depresyon durumuna göre basit rastgele örnekleme yöntemiyle seçilen postpartum depresyonu olan 147 kadın (vaka grubu) ve postpartum depresyonu olmayan 147 kadın (kontrol grubu) yer almıştır. Araştırmanın verileri, araştırmacı tarafından aile sağlığı merkezinde uygun bir görüşme odasında kadınlarla yüz yüze görüşülerek ve Kişisel Bilgi Formu, Fagerström Nikotin Bağımlılık Testi ve Edinburg Postpartum Depresyon Ölçeği kullanılarak toplanmıştır. Kontrol grubundaki kadınların %51.6’sının, vaka grubundaki kadınların %51.9’unun sigara içtiği, kontrol grubundaki kadınların %16.3’ünün, vaka grubundaki kadınların ise %4.6’sının bağımlılık düzeylerinin çok yüksek olduğu belirlenmiştir. Vaka grubunun Edinburg Postpartum Depresyon Ölçeği toplam puan ortalaması ile Fagerstrom Nikotin Bağımlılık Testi toplam puan ortalaması arasında anlamlı, pozitif yönde ve zayıf düzeyde bir ilişki bulunmuştur (p<0.05). Lojistik regresyon analizi nikotin bağımlılık düzeyi çok yüksek olan kadınların, nikotin bağımlılık düzeyi çok düşük olan kadınlara göre postpartum depresyon görülme olasılığının 7.34 kat daha fazla olduğunu ortaya koymuştur. Vaka ve kontrol grubundaki kadınların sigara içme durumları ile postpartum depresyon arasında ilişki olmadığı, ancak vaka grubundaki kadınların depresyon düzeyleri arttıkça nikotin bağımlılık düzeylerinin arttığı ortaya çıkmıştır.
