Effects of Different Remineralization Gels on Post-Orthodontic White Spot Lesions: A 4-Week Follow-Up Clinical Trial

Abstract

Purpose: This in vivo study assessed and compared the remineralizing effects of gels containing Calcium Glycerophosphate (CaGP) and Casein Phosphopeptide-Amorphous Calcium Phosphate (CPP-ACP) on white spot lesions (WSLs) after orthodontic treatment. Materials and Methods: A total of 67 teeth from five patients (aged 19–21) with post-orthodontic WSLs were included. Patients were allocated into two groups: Group 1 received a CaGP-containing gel, and Group 2 received a CPP-ACP-containing gel. Both groups used reservoir trays to apply the gels for 5 minutes twice daily over four weeks. Evaluation methods included the Quantitative Light-Induced Fluorescence (QLF), Laser Fluorescence (LF) measurements and the Modified Gorelick Index, assessed at preintervention and postintervention. Results: QLF analysis showed a reduction in lesion depth (ΔF), area (A), and lesion volume (ΔQ) in both groups. A significant improvement in ΔF was found only in the CaGP group (p=0.003), while significant reductions in lesion area and ΔQ were observed only in the CPP-ACP group (p=0.030, p=0.023). LF values decreased significantly in both groups (p<0.001), with a greater reduction in the CPP-ACP group. Both groups also showed significant clinical improvement in Modified Gorelick scores. Both gels showed positive effects in the remineralization of WSLs. Conclusions: These findings suggest that both CaGP and CPP-ACP gels, when applied with reservoir trays, may be effective adjuncts for the remineralization of post-orthodontic WSLs.
