Surgical Retrieval of Mandibular Thirt Molar Root Displaced İnto the Pterygomandibular Space

Öz

Today, impacted mandibular third molar surgery has become a routine procedure in oral and maxillofacial surgery. One of the numerous complications which can occur during the procedure is iatrogenic tooth or tooth fragment displacements into the neighboring soft tissues such as the submandibular space, the pterygomandibular space and the lateral pharyngeal space. The dentist should be able to recognize this complication and refer the patient to an oral surgeon. In this case report we present an iatrogenic root displacement into the pterygomandibular space during impacted third molar surgery. In the 31 year old female patient, lingual mucosa flap was used for retrieval surgery, fragment was raised upwards with external hand pressure beneath the mandibula, directed to the floor of the mouth. The tooth fragment was then visualized with dissection using blunt tissue scissors and retrieved. Wound was sutured and antibiotics, analgesics and mouth wash was prescribed. Healing was uneventful with no lingual nerve complication

PTERYGOMANDİBULER LOJA KAÇAN ALT YİRMİ YAŞ DİŞ KÖKÜNÜN CERRAHİ OLARAK ÇIKARILMASI

Öz

Gömülü alt yirmi yaş dişi çekimi günümüzde ağız, diş ve çene cerrahisi alanında rutin olarak uygulanan bir işlem haline gelmiştir. Bu işlem sırasında meydana gelebilecek birçok komplikasyondan biri de çekilen dişin ya da dişin bir bölümünün submandibuler loj, pterygomandibuler lojya da lateral faringeal loj gibi komşu yumuşak dokulara kaçması-itilmesidir. Bu komplikasyon meydana geldiği zaman hekimin durumu tanıyabilmesi ve maxsillafasial cerraha yönlendirmesi gerekmektedir. Vaka raporumuzda gömülü alt yirmi yaş dişi çekimi sırasında, pterygomandibular loja deplase olan bir kökün cerrahi olarak çıkartılması paylaşılmıştır. 31 yaşındaki kadın hastada kökün çıkartılması için lingual mukoza flebi kaldırılmış, mandibula altından ağız tabanına doğru uygulanan ekstraoral bası ile kök ağız içerisine doğru yükseltilmiştir. Künt doku makasları ile yapılan diseksiyonla kök parçasına ulaşılarak çıkartılmıştır. Yara dikilmiş, antibiyotik, ağrı kesici ve ağız gargarası reçete edilmiştir. İyileşme lingual sinir hasarı gelişmeden, komplikasyonsuz olarak gerçekleşmiştir.
