Unexpected Nasal Cavity Entry of a Maxillary Sinus Implant During Intraoperative Removal

Abstract

Dental implant displacement into the maxillary sinus is a well-documented complication in implantology. This case report presents a rare intraoperative event where a dental implant, initially displaced into the maxillary sinus due to insufficient bone support, migrated into the nasal cavity during surgical retrieval. A 45-year-old female patient with chronic maxillary sinusitis and an existing oroantral fistula underwent surgical treatment using the Caldwell-Luc approach. Despite thorough sinus debridement, the implant could not be located intraoperatively, and a cone-beam computed tomography (CBCT) scan revealed its unexpected presence in the nasal cavity, near the ostium. The implant was successfully retrieved through the sinus window, and the fistula was closed using a buccal fat pad. This case highlights the need for meticulous intraoperative assessment and readiness for atypical complications such as nasal migration of implants, particularly in patients with prolonged sinus pathology.
