Laboratory Error Masquerading as Toxic Alcohol Ingestion: A Teaching Case Report

Abstract

Introduction: Elevated osmolar gap is strongly associated with toxic alcohol ingestion, often triggering reflexive interventions including emergent hemodialysis. Clinical-laboratory discordance should prompt critical re-evaluation before invasive procedures. Case Presentation: A 51-year-old woman presented with altered mental status and an osmolar gap of 427 mOsm/kg with high anion gap metabolic acidosis, initially suggesting toxic alcohol ingestion. Critical evaluation revealed that the primary disturbance was respiratory acidosis, the patient responded to naloxone, and the implied alcohol concentrations would be incompatible with survival. A repeat osmolality was 314 mOsm/kg, uncovering a transcription error: the laboratory technician had entered the specimen collection time (7:08 AM) as '708' instead of the true osmolality (310 mOsm/kg). The patient recovered with supportive management for opioid overdose and acute kidney injury, without hemodialysis. Conclusion: Clinicians must critically reassess when laboratory results are discordant with the clinical picture. A brief pause before irreversible interventions upholds patient safety and resource stewardship.

Toksik Alkol Alımını Taklit Eden Laboratuvar Hatası: Bir Öğretici Olgu Sunumu

Abstract

Giriş: Yüksek osmolar açık, toksik alkol alımıyla güçlü bir şekilde ilişkilendirilmekte ve sıklıkla acil hemodiyaliz dahil refleks müdahaleleri tetiklemektedir. Klinik-laboratuvar uyumsuzluğu, invaziv prosedürler öncesinde kritik bir yeniden değerlendirmeyi zorunlu kılmalıdır. Olgu Sunumu: Elli bir yaşında bir kadın hasta, değişmiş mental durum ile başvurdu ve yüksek anyon açıklı metabolik asidoz eşliğinde 427 mOsm/kg osmolar açığı saptandı; bu bulgular başlangıçta toksik alkol alımını düşündürdü. Kritik değerlendirme; primer bozukluğun respiratuar asidoz olduğunu, hastanın naloksona yanıt verdiğini ve hesaplanan alkol konsantrasyonlarının yaşamla bağdaşmayacak düzeyde olduğunu ortaya koydu. Tekrarlanan osmolalite ölçümü 314 mOsm/kg olarak bulundu ve bir transkripsiyon hatası gün yüzüne çıktı: laboratuvar teknisyeni, numune toplama saatini (07:08) gerçek osmolalite değeri (310 mOsm/kg) yerine '708' olarak kaydetmişti. Hasta, opioid aşırı dozu ve akut böbrek hasarı için hemodiyaliz uygulanmadan destekleyici tedaviyle iyileşti. Sonuç: Klinisyenler, laboratuvar sonuçlarının klinik tablo ile uyumsuz olduğu durumlarda, klasik öğretim kalıplarıyla karşılaşıldığında dahi, eleştirel bir yeniden değerlendirme yapmalıdır. Geri dönüşümsüz girişimler öncesinde kısa bir duraksamak, hasta güvenliğini ve kaynak yönetimini destekler.
