Tianeptin (İlaç Kötüye) Kullanımına Bağlı Toksik Lökoensefalopati: Olgu Sunumu

Abstract

Neurological complications of drug and substance abuse include cerebral white matter damage (leukoencephalopathy). Toxic leukoencephalopathy (TL) is a cerebral white matter disorder caused by exposure to leukotoxic agents. Acute toxic leukoencephalopathy (ATL) is a potentially reversible condition that may resolve after toxin withdrawal after treatment or early in the disease course. The most common causes of ATL in adults include chemotherapeutic agents, immunosuppressant therapy, illicit drug use, and drug overuse, particularly opioid overuse. It has been reported in the literature that overuse and abuse of benzodiazepines and some antidepressants cause toxic leukoencephalopathy.

Abstract

İlaç ve madde kötüye kullanımının nörolojik komplikasyonları arasında serebral beyaz cevher hasarı (lökoensefalopati) yer alır. Toksik lökoensefalopati (TL) lökotoksik ajanlara maruz kalmanın neden olduğu serebral beyaz cevher bozukluğudur. Akut toksik lökoensefalopati (ATL), tedaviden sonra veya hastalık seyrinin erken döneminde toksinin kesilmesinin ardından düzelebilen potansiyel olarak geri dönüşümlü bir durumdur. Yetişkinlerde ATL'nin en yaygın nedenleri arasında kemoterapötik ajanlar, immünosupresan tedavi, yasadışı ilaç kullanımı ve özellikle opioid aşırı kullanımından kaynaklanan aşırı ilaç kullanımı yer alır. Benzodiazepin ve bazı antidepresanların aşırı ve kötüye kullanımının toksik lökoensefalopatiye neden olduğu literatürde bildirilmiştir.
