Sınırlı Ağız Açıklığı Olan Bir Hasta İçin Ataçmanlı Teleskobik Protez: Olgu Sunumu

Abstract

Herhangi bir nedene bağlı olarak ağız açıklığı kısıtlılığına sahip hastalarda protetik rehabilitasyon, hem diş hekimi hem de hastalar için zorluklar ortaya çıkarmaktadır. Bu hastalar için özel ölçü teknikleri kullanmak ve özel tasarım protezler yapmak gerekmektedir. Bu çalışmanın amacı, maksiller hassas ataşmanla ayrılabilen teleskop tutuculu overdenture protez ve mandibular hareketli bölümlü protez yapımı ile ağız açıklığı kısıtlı bir hastanın rehabilitasyonunu değerlendirmektir. Ağız açıklığı sınırlı 27 yaşında erkek hasta, Ordu Üniversitesi Protetik Diş Tedavisi Anabilim Dalı'na protez yaptırmak için başvurmuştur. Yapılan muayenede hastanın ağzında bulunan maksiller dişlerin teleskop protez yapımı için uygun olduğu gözlenmiştir. Ölçüler için parçalı ölçü tekniğine uygun olarak hazırlanan özel kaşıklar, iki parçadan üretilmiştir. Üst çene için parçalı çıkarılabilir diş protezleri, protezin hastanın ağzına rahat takılabilmesi için orta hat üzerinde hassas ataşmanlı olarak tasarlanmıştır. Parçalı protezler ağıza takıldıktan sonra yerine kilitlenebilir özelliktedir. Protezler takıldıktan sonra 1. ayda ve 6. ayda değerlendirilmiş ve herhangi bir komplikasyonla karşılaşılmamıştır. Sunulan protokol, sınırlı ağız açıklığı olan hastalar için uygun bir tedavi seçeneği sağlayabilir. Ancak, her hastanın ihtiyacına göre tedavi planlaması yapmak gereklidir. Ayrıca, tedavi prosedürünün sınırlamaları ve sürekli bakım gereksinimi hastaya vurgulanmalıdır.

Telescopic Prosthesis with Attachment for a Patient with Limited Mouth Opening: A Case Report

Abstract

Prosthetic rehabilitation of patients with limited oral opening due to any causative factors, presents difficulties for both dentists and patients. It is necessary to use special impression techniques and to fabricate special design prostheses for these patients. The aim of this study was to evaluate the rehabilitation of a patient with limited oral opening with maxillary telescobic overdenture prosthesis separated by precision attachment and mandibular removable partial prosthesis. A 27-year-old male patient with limited oral opening applied to Ordu University Department of Prosthodontics for a prosthesis. Various treatment options were discussed, and treatment was decided with the patient. Sectional trays prepared for the sectional impression technique and maxillary and mandibular impressions were made. The maxillary denture base was prepared in 2 pieces for easy insertion of the prosthesis into the patient's mouth. These pieces were joined by precision attachment in the midline. Prostheses can be locked in place after they are inserted into the mouth. Recalls have been completed at 1.day, 1.week, 1.month and 6.month. The patient complained were identified and eliminated. As a result, the dentures have been functioning well and an acceptable fit has been established. The presented protocol may provide an appropriate treatment option for patients with limited oral opening. However, it is necessary to plan the treatment according to the needs of a patient. Furthermore, the limitations of the treatment procedure and the need for continuous care should be emphasized to the patient.
