Kronik Obstrüktif Akciğer Hastalığı Anksiyete Ölçeğinin Türkçe Geçerlik ve Güvenirlik Çalışması

Abstract
Amaç: Bu araştırma, Kronik Obstrüktif Akciğer Hastalığı (KOAH) Anksiyete Ölçeğinin Türkçe geçerliğini ve güvenirliğini belirlemek amacıyla yürütüldü.
Materyal ve Metot: Metodolojik türde olan araştırmanın örneklemini Atatürk Üniversitesi Sağlık Araştırma ve Uygulama Merkezi ve Erzurum Şehir Hastanesi göğüs hastalıkları kliniğinde tedavi gören 401 hasta oluşturdu. Veri toplama araçları olarak, Tanımlayıcı Özellikler Formu, KOAH Anksiyete Ölçeği, KOAH Değerlendirme Testi, Modified Medical Research Council Dyspnea Skalası (mMRC), ve Hastane Anksiyete ve Depresyon Ölçeği kullanıldı. Madde toplam korelasyonu, açıklayıcı ve doğrulayıcı faktör analizi, Kaiser-Meyer-Olkin testi ve Bartlett's Küresellik testi, test-tekrar test güvenirliği, paralel formlar güvenirliği, Cronbach alfa katsayısı hesaplandı.
Bulgular: Ölçekte bulunan maddelerin madde toplam korelasyon değerlerinin 0.36-0.64 arasında olduğu saptandı. KMO değerinin 0.854 olduğu, Bartlett's Küresellik testinin anlamlı olduğu saptandı (p<0.05). Ölçeğin 18 madde ve beş alt boyuttan oluştuğu saptandı. Ölçeğin açıklanan varyans oranının %81.596 olduğu görüldü. Doğrulayıcı faktör analizi uyum indekslerinin CFI=0.950, RMSEA=0.078 olduğu belirlendi. Ölçeğin Cronbach Alpha katsayısının 0.90, alt boyutlarının Cronbach Alpha katsayılarının 0.78 ile 0.93 arasında olduğu saptandı. Ölçeğin zamana karşı değişmezliğinde kararlı bir ölçüm aracı olduğu bulundu. KOAH Anksiyete Ölçeği ve alt boyutları ile KOAH Değerlendirme Testi, Modified Medical Research Council Dispne Skalası (mMRC), Hastane Anksiyete ve Depresyon Ölçeği arasında pozitif yönde bir ilişki olduğu belirlendi (p<0.05).
Sonuç: KOAH Anksiyete Ölçeğinin Türkçe versiyonunun geçerli ve güvenilir bir ölçüm aracı olduğu, KOAH’a özgü anksiyetenin değerlendirilmesinde kullanılabileceği sonucuna varıldı.

Abstract
Objective: This study was conducted to determine the Turkish validity and reliability of the Chronic Obstructive Pulmonary Disease (COPD) Anxiety Scale.
Methods: The sample of the methodological study consisted of 401 patients treated in the Atatürk University Health Research and Application Centre and the Erzurum City Hospital chest diseases clinic. The Descriptive Characteristics Form, COPD Anxiety Scale, COPD Assessment Test (CAT), Modified Medical Research Council Scale (mMRC), and Hospital Anxiety and Depression Scale (HADS) were used as data collection tools. Item-total correlation, exploratory and confirmatory factor analysis, Kaiser-Meyer-Olkin test, Bartlett's Sphericity test, test-retest reliability, parallel forms reliability, and Cronbach's alpha coefficient were calculated.
Results: Item-total correlations ranged from 0.36 to 0.64. The Kaiser-Meyer-Olkin value was 0.854, and Bartlett’s test was significant (p<0.05). The scale consists of 18 items and 5 sub-dimensions. The total explained variance was 81.596%. Confirmatory factor analysis fit indices were CFI=0.950 and RMSEA=0.078. Cronbach’s alpha was 0.90 for the whole scale and between 0.78 and 0.93 for the sub-dimensions. The scale was found to be a stable measurement tool with time invariance. The scale and its sub-dimensions showed positive correlations with CAT, mMRC, and HADS (p<0.05). The COPD Anxiety Scale and its sub-dimensions showed positive correlations with CAT, mMRC, and HADS (p<0.05).
Conclusion: The Turkish version of the COPD Anxiety Scale is a valid and reliable measurement tool that can be used to evaluate COPD-specific anxiety.
