                                                                                                <abstract><p>Amaç: Bu araştırmada, Erzurum Şehir Hastanesinde elektronik olay bildirim sistemi üzerinden raporlanan hasta düşmelerinin nedenleri, kaynakları ve kök nedenlerinin incelenmesi amaçlanmıştır.Yöntem: Tanımlayıcı tipteki araştırmanın evrenini, 2023 yılı boyunca Hastanenin kalite yönetim sistemi aracılığıyla bildirilen tüm düşme olayları oluşturmuştur. Örnekleme gidilmeksizin evrenin tamamına ulaşılmış ve toplam 35 düşme olayı incelenmiştir. Veriler, Düşme Olayı Bildirim Formu aracılığıyla toplanmış; hastaların demografik bilgileri, klinik özellikleri, düşmenin gerçekleştiği zaman, olayın kaynağı ve nedeni değerlendirilmiştir. Veriler, IBM SPSS Statistics 29.0 paket programı ile analiz edilmiştir.Bulgular: Çalışma kapsamında raporlanan 35 düşme olayının %51,4’ü erkeklerde, %48,6’sı kadınlarda gerçekleşmiştir. Hastaların yaş ortalaması 68,8±13,4’tür. Düşmelerin %68.6’sının servislerde, %31.4’ünün ise yoğun bakım ünitelerinde meydana geldiği; olayların %68,6’sının hasta kaynaklı olduğu belirlenmiştir. Düşmeler en sık 12:00–18:00 ve 18:00–24:00 saatleri arasında (%57,2) görülmüş; nedenler arasında denge kaybı/bayılma (%31,4), yatakta ajitasyon (%17,1) ve yataktan inme (%14,3) öne çıkmıştır. Ortalama düşme risk skoru 13,25±8,03’tür.Sonuç: Bulgular, ileri yaş ve kronik hastalık varlığına sahip bireylerin düşme açısından daha yüksek risk altında olduğunu göstermektedir. Dahili kliniklerde riskin yüksek olması, bu alanlarda hemşirelik bakımının güçlendirilmesini gerekli kılmaktadır. Özellikle öğleden sonra ve akşam saatlerinde önleyici girişimlerin artırılması ve elektronik bildirim sistemlerinin etkin kullanılması, düşme olaylarının azaltılmasına katkı sağlayacaktır.</p></abstract>

                            <p>Objective: This study aimed to investigate the causes, sources, and underlying factors contributing to patient falls reported through the electronic incident reporting system at Erzurum City Hospital.Methods: This descriptive study included all patient falls reported in 2023 through the hospital’s quality management system. Without sampling, a total of 35 fall incidents were evaluated. Data were collected using the Fall Incident Notification Form, which includes demographic characteristics, clinical information, time of the fall, source, and cause of the incident. Data were analysed using IBM SPSS Statistics 29.0 software.Results: Of the 35 reported falls, 51.4% occurred in male patients and 48.6% in female patients. The mean age of the patients was 68.8±13.4 years. It was determined that 68.6% of the falls occurred in wards and 31.4% in intensive care units, and that 68.6% of the incidents were patient-related. Falls were most frequent between 12:00–18:00 and 18:00–24:00 (57.2%). The main causes were loss of balance/fainting (31.4%), agitation in bed (17.1%), and getting out of bed (14.3%). The mean fall risk score was 13.25±8.03.Conclusion: The findings suggest that elderly patients with chronic diseases are at a higher risk of falls. The high incidence in internal medicine clinics highlights the need to strengthen nursing practices in these units. Preventive measures should be reinforced, especially in the afternoon and evening hours, and the effective use of electronic reporting systems may contribute to reducing hospital falls.</p></trans-abstract>
