The Relationship Between Fear of Falling, Kinesiophobia, and Dual-Task Performance in Chronic Stroke: A Study on Self-Reported and Performance-Based Measures

Abstract

Purpose: The aim of this study was to evaluate the relationship between Fear of Falling (FoF), kinesiophobia and both performance-based and self-reported dual task in patients with chronic stroke and compare their results with healthy individuals. Method: The study consisted of 37 patients diagnosed with chronic stroke and 30 healthy individuals as the control group. Self-reported dual-task was evaluated using the Dual-Task Questionnaire, whereas performance-based single- and dual-task assessments were conducted with the Timed Up and Go (TUG), TUG-Motor, and TUG-Cognitive tasks. FoF was measured with the Falls Efficacy Scale–International, and kinesiophobia was assessed using the Tampa Scale of Kinesiophobia. Results: Patients with chronic stroke showed higher levels of FoF and kinesiophobia, alongside lower self-reported dual-task performance compared with healthy controls (p≤0.001). They also showed poorer performance on all TUG-based assessments (TUG, TUG-Motor and TUG-Cognitive) (p<0.001). Moderate positive correlations were identified between FoF and both dual-task performance and kinesiophobia (r=0.477, p=0.003; r=0.532, p=0.001). Dual-task performance was similarly moderately correlated with kinesiophobia and TUG outcomes (r=0.403–0.465, p≤0.016). In contrast, no significant associations were found between fear of falling, kinesiophobia, and TUG measures (p>0.05). Discussion: In conclusion, it was determined that individuals with chronic stroke have significant deficits in fear of falling, kinesiophobia, and dual-task performance, and that these impairments correlate with each other. The findings highlight the importance of psychosocial and cognitive factors on physical performance in stroke patients.
