Cranial Anthropometric Changes and Their Relationship With Parental Expectations and Satisfaction Following Cranial Remolding Orthosis Treatment in Infants With Positional Cranial Deformities

Abstract

Purpose: The study examined the relationship between cranial measurements, parental expectations and satisfaction levels, and changes in cranial measurements during treatment with a cranial orthosis. Method: The study included 10 infants diagnosed with positional cranial deformities and their parents. The infants wore cranial orthoses for a month and cranial deformity was determined using the Cranial Index (CI) and the Cranial Vault Asymmetry Index (CVAI). The Treatment Expectation Questionnaire (TEX-Q) and The Quebec User Evaluation of Satisfaction with Assistive Technology (QUEST 2.0) questionnaires were used to assess parental expectations and satisfaction regarding treatment. Changes in cranial deformity between baseline and post-treatment were analysed using the Wilcoxon signed-rank test. The correlations between cranial anthropometric measurements and TEX-Q and QUEST scale scores were analysed using the Spearman’s rank correlation coefficient. Results: The average age at the start of the treatment was 5.4 ± 2.11 months. Cranial deformities in infants have decreased significantly (p=0.017) after the treatment. Furthermore, there was a moderate positive correlation between baseline CVAI values and the age of infants (r=0.640, p=0.046). A strong negative correlation was found between TEX-Q scores and pre-treatment CVAI (r=-0.737; p=0.015). No significant correlation was found between QUEST scores and any cranial measurements (p>0.05). Discussion: Although cranial orthotic treatment is effective in correcting positional cranial deformities, the success of the treatment depends on more than just anthropometric improvement. It also depends on a holistic approach that considers parental expectations, device usability, and psychosocial factors.
