Osaslı Hastalarda Gün İçi Artmış Uykululuk Hali ve Depresyon

Abstract

ÖZET
Amaç: Depresyonun, gün içi artmış uykululuk hali ve obstrüktif uyku apne sendromu (OSAS) ile ilişkisi çeşitli çalışmalarda incelenmiş, ancak bu ilişki tam olarak netleşmemiştir. Bu çalışmada, OSAS şüphesiyle polisomnografi (PSG) yapılan hastalarda depresyon skorları değerlendirilmiş ve gün içi uykululuk halinin bu durumlarla ilişkisi araştırılmıştır.
Yöntem: PSG yapılan 18–75 yaş aralığında, toplam 103 hasta (71 erkek, 30 kadın) retrospektif olarak değerlendirildi. Hastaların komorbiditeleri, vücut kitle indeksleri (VKİ), Epworth Uykululuk Skalası (ESS) ve Beck Depresyon İndeksi (BDİ) verileri incelendi. Psikiyatrik hastalar dışlandı.
Bulgular: Hafif OSAS 29 hastada, orta OSAS 24 hastada, ağır OSAS ise 42 hastada saptandı. BDİ’ye göre 69 hasta depresyonla uyumlu skor alırken, 32 hasta non-depresif kabul edildi. ESS puanı 10’un üzerinde olan hastalarda gün içi uykululuk hali var kabul edildi. AHI ile BDİ arasında anlamlı ilişki bulunmazken (p=0,390), AHI ile ESS arasında anlamlı ilişki saptandı (p=0,005). ESS ile BDI arasında ise ilişki izlenmedi (p=0,301).
Sonuç: Gün içi artmış uykululuk hali ile depresyon arasında doğrudan bir ilişki gözlenmemiştir. OSAS’lı hastalarda duygudurumun dikkatle değerlendirilmesi gerektiği sonucuna varılmıştır. Daha geniş örneklemli çalışmalara ihtiyaç vardır.

Excessive Daytime Sleepiness And Depressıon in Patıents with Osas

Abstract

Objective: The relationship between depression, excessive daytime sleepiness (EDS), and obstructive sleep apnoea syndrome (OSAS) has been investigated in various studies, but this relationship remains unclear. In this study, depression scores in patients undergoing polysomnography (PSG) for suspected OSAS were evaluated, and the relationship between daytime sleepiness and these conditions was investigated.
Method: A total of 103 patients (71 men, 30 women) aged 18–75 years who underwent PSG were retrospectively evaluated. Comorbidities, body mass index (BMI), Epworth Sleepiness Scale (ESS), and Beck Depression Inventory (BDI) data were analysed. Patients with psychiatric disorders were excluded.
Results: Mild OSAS was found in 29 patients, moderate OSAS in 24 patients, and severe OSAS in 42 patients. According to the BDI, 69 patients had scores consistent with depression, while 32 patients were considered non-depressed. Patients with ESS scores above 10 were considered to have excessive daytime sleepiness. No significant relationship was found between AHI and BDI (p=0.390), but a significant relationship was found between AHI and ESS (p=0.005). No significant relationship was observed between ESS and BDI (p=0.301).
Conclusion: No direct relationship was observed between excessive daytime sleepiness and depression. It was concluded that the emotional state of patients with OSAS should be evaluated carefully. Further studies with larger samples are needed.
