Pubertal Yaştaki Juvenil İdiyopatik Artritli Hastalarda Kemik Yaşının Değerlendirilmesi

Öz

Amaç: Kronik hastalıklarda büyümenin ve iskelet maturasyonunun geri kaldığı bilinmektedir. Kronik sistemik bir hastalık olan ve uzun süre yüksek doz NSAİİ, steroid ve immunsupresif ilaçların kullanıldığı Juvenil İdiyopatik Artrit’in JİA büyüme üzerine olumsuz etkisi mevcutur. Büyümeyi sağlayan en önemli faktör iskelet büyümesidir. Kemik yaşı iskelet maturasyonunun bir göstergesi olup, kronolojik yaşa göre ±2 yıl farklılık gösterebilir. Bu çalışmada JİA’li hastalarda kemik yaşının nasıl etkilendiği araştırmak amaçlanmıştır. Yöntemler: Çalışmamıza S.B. İstanbul Göztepe Eğitim ve Araştırma Hastanesi Çocuk Sağlığı ve Hastalıkları Romatoloji Polikliniği’nde, hastalığın başlangıcından itibaren izlenip düzenli kontrolleri yapılan, ILAR teşhis kriterlerine göre JİA tanısı almış en az iki yıl takipli hastalar çalışma gurubuna alındı. Bulgular: Çalışmaya 50 JİA’li çocuk, 22 erkek, 28 kız dahil edildi. Hastaların sol el bilek grafileri çekilip Greulich-Pyle atlası kullanılarak kemik yaşları tespit edildi. Hastaların ortalama takvim yaşı 12.11±2.05 yaş, ortalama kemik yaşı ise 12.3±2.19 yaş saptandı. Aralarında istatiksel olarak anlamlı fark saptanmadı. Hastaların 47’sinde % 94 kemik yaşı ±2 yıl içinde saptandı. Hastaların 3’ünde % 6 kemik yaşı ileri 2 yıldan fazla saptandı. Hiçbir hastada kemik yaşı geri saptanmadı. Sonuç: Sonuç olarak, bulgular JİA’te hastalığın iskelet maturasyonuna olan etkisinin somatik büyümeden daha az olduğunu düşündürdü.

Evaluation of Bone Age of Patients with Juvenile Idiopathic Arthritis in Puberty

Objective: It’s known that growth and skeletal maturation are retarded in chronic diseases. Juvenile idiopathic arthritis is a chronic disease and is treated with NSAİDs, steroids and immunosupressive medications for a long time, and its influence on growth is known. The most important factor to maintain growth is skeletal development. Bone age is an indicator of skeleteal maturation. Bone age can show ±2 year difference compared with chronological age. The aim of this study is to evaluate how bone age is affected in patients with JIA. Methods: Patients who were diagnosed with JIA according to ILAR diagnostic criteria and who were followed up from the beginning of the disease in Ministry of Health Goztepe E.and R.H. Pediatric Rheumatology Clinic were included in the study. Results: Fifty children 22 males, 28 females with JIA were studied. Left wrist roentgenograms were taken and bone ages were determined according to Greulich-Pyle atlas. The mean chronological age of the patients was 12.11±2.05 years, the mean bone age was 12.3±2.19 years. There wasn’t any statistically significant difference in between. Bone age was found in ±2 year in 47 patients 94 % . Bone age was found ahead more than 2 years in 3 6 % patients. Bone age wasn’t found to be retarded in any patients.
