The role of first-trimester hematological parameters in predicting clinically significant postpartum hemoglobin decline: a retrospective study from a perinatology unit

Abstract

Aims: To evaluate the role of first-trimester hematological parameters in predicting clinically significant postpartum hemoglobin decline. Methods: This retrospective study included 320 pregnant female who delivered at the Perinatology Unit of Şanlıurfa Training and Research Hospital. Participants were divided into two groups according to postpartum hemoglobin decline; <2 g/dl (n=160) and ≥2 g/dl (n=160). Maternal demographic characteristics, obstetric data, first-trimester complete blood count parameters, and delivery and postpartum outcomes were analyzed. Variables associated with clinically significant postpartum hemoglobin decline were assessed using logistic regression analysis, and predictive performance was evaluated by receiver operating characteristic analysis. Results: Female with postpartum hemoglobin decline ≥2 g/dl had lower first-trimester hemoglobin and hematocrit levels and higher red cell distribution width and Mentzer Index values. This group also had significantly higher rates of cesarean delivery, estimated blood loss, postpartum hemorrhage, additional uterotonic requirement, red blood cell transfusion, and longer hospital stay. In multivariable analysis, lower first-trimester hemoglobin, higher red cell distribution width, higher Mentzer Index, and cesarean delivery remained independent predictors. The combined model showed better discriminative performance than any single marker. Conclusion: First-trimester hematological parameters, particularly hemoglobin, red cell distribution width, and Mentzer Index, are associated with clinically significant postpartum hemoglobin decline. These readily available markers may contribute to early obstetric risk stratification.
