Aterosklerotik koroner arter hastalığı ile karışan koroner vasospazm olgusu

Abstract

Akut koroner sendrom tanısı ile hastaneye başvuran hastaların bir çoğunda aterosklerotik tıkayıcı koroner arter hastalığı saptanırken bir kısmında özellikle genç bayan hastalarda koroner damarların açık olduğu saptanmaktadır. Bu hastaların bir kısmında ise koroner vasospazm saptanmaktadır. Koroner vasospazm hastaları sadece göğüs ağrısından ciddi akut koroner sendrom kliniğine kadar ciddi klinik durumlarla başvurabilmektedir. Bu tür hastalar bazen tıkayıcı aterosklerotik koroner arter hastalığı gibi tedavi edilmektedir. Hastalığın tanısının doğru konulması ve tedavinin doğru yapılması için koroner anjioğrafi esnasında saptanan darlıkların intrakoroner nitrat verilerek kontrol edilmesi uygun olacaktır.

A case of coronary vasospasm confused with atherosclerotic coronary artery disease

Abstract

While most patients presenting to the hospital with the diagnosis of acute coronary syndrome have atherosclerotic obstructive coronary artery disease, some of them are open to coronary vessels especially in young female patients. Coronary vasospasm is detected in some of these patients. Patients with coronary vasospasm can only be admitted with severe clinical conditions from chest pain to severe acute coronary syndrome. Such patients are sometimes treated as occlusive atherosclerotic coronary artery disease. For correct diagnosis of the disease and correct treatment, it is appropriate to control the stenosis detected during coronary angiography by providing intracoronary nitrate.
