Evaluation of pneumococcal and influenza vaccination rates and patients' attitudes towards these vaccines in hemodialysis patients

Abstract

Aims: To assess pneumococcal and influenza vaccination rates among haemodialysis patients, and to evaluate their attitudes and beliefs regarding these vaccines. Methods: Between 20 January and 20 February 2024, a cross-sectional study was conducted across private dialysis centres in Ankara, Turkiye. A total of 200 patients undergoing haemodialysis for chronic kidney disease were included in the study. A face-to-face questionnaire administered by dialysis physician routine sessions captured demographic characteristics, vaccination status and reasons for vaccine hesitancy. Vaccination status was recorded as ever (yes/no); exact timing was not analysed. Descriptive statistics and comparative analyses were performed using IBM SPSS Statistics v22.0; two-sided p<0.05 was considered statistically significant. Results: Among 200 haemodialysis patients, 66.5% reported ever receiving an influenza vaccine and 22.5% reported ever receiving a pneumococcal vaccine. Belief in the efficacy of the influenza vaccine was notably higher (63.5%) than that of the pneumococcal vaccine (24%). The most frequently cited reasons for not being vaccinated were fear of side effects and a perceived lack of benefit. In bivariable analyses, greater vaccine acceptance was significantly associated with older age, higher educational status, and smoking history (p<0.05). Recommendations for vaccination were predominantly made by specialists rather than general practitioners. Conclusion: Vaccination rates, particularly for the pneumococcal vaccine, remain low among haemodialysis patients. Poor uptake is due to misconceptions and lack of awareness. Improving immunisation coverage in this high-risk group could be achieved through targeted patient education and proactive recommendations by healthcare providers.
