Virtual dissection: a comprehensive analysis of YouTube videos on transabdominal preperitoneal (TAPP) inguinal hernia repair

Abstract

Aims: YouTube has become an increasingly popular visual and audio resource in modern surgical education and plays an important role, particularly in the learning of laparoscopic techniques. This study aims to evaluate the use of transabdominal preperitoneal (TAPP) inguinal hernia repair videos on YouTube in surgical education and to analyze the changes in the quality of education seen in these videos over time. Methods: On September 9, 2024, a search was conducted on YouTube using the term "TAPP inguinal hernia repair," and among the 100 most viewed videos, 54 videos that met the inclusion criteria were examined. Videos were divided into two groups based on upload date: those uploaded in the last five years (group B) and those older (group A). Criteria, such as video length, view count, number of likes and comments, LAP-VEGaS and GOALS-GH scores, were compared. Mann-Whitney U test and Chi-square test were used for statistical comparisons. Results: 59.2% of the videos (32 videos) were in group A. There was no difference in terms of video length, view count, and views per day. While the number of likes was higher in group A (p=0.02), the interaction rate was significantly higher in group B (p=0.001). In terms of educational quality, group B was found to be statistically significantly better in both LAP-VEGaS (p=0.01) and GOALS-GH scores (p=0.01). Particularly, trocar placement, peritoneal flap creation, and hernia sac dissection were statistically significantly better in group B. Additionally, image quality was better in group B (p=0.02). No difference was detected in terms of voice and upload source. Conclusion: The findings obtained in this study suggest that TAPP hernia surgery videos on YouTube are valuable resources in surgical education and that the educational quality of these videos has improved over time. However, to increase the impact of YouTube in surgical education, it is recommended that the platform enhance its content filtering systems to prioritize scientific accuracy and educational value.
