A retrospective assessment of HIV, HBV, and HCV seroprevalence and concordance between patient self-reported status and serological findings in oral surgery patients

Abstract

Aims: This study aimed to evaluate the seroprevalence of HBV, HCV, and HIV in patients undergoing oral surgery and to assess concordance between patient self-reports and serological results. Additionally, occupational distribution, dental attendance, and family history of seropositive patients were analyzed. Methods: As part of the institutional infection control protocol, all patients scheduled for invasive oral surgical procedures under local anesthesia routinely underwent preoperative serological screening. This retrospective study included 3968 patients who underwent oral surgical procedures at the Department of Oral and Maxillofacial Surgery at Adıyaman University between March 2025 and March 2026. Preoperative blood test results, including HBsAg, anti-HCV, and anti-HIV, were retrospectively retrieved from archived laboratory records. Concordance between patient self-reported medical history and serological findings was evaluated. In seropositive patients, occupational status, dental attendance, and family history of HBV, HCV, or HIV infection were also analyzed. Results: A total of 3968 patients were included (41.2% male, 58.8% female). HBsAg, anti-HCV, and anti-HIV positivity rates were 0.96%, 0.20%, and 0.05%, respectively, with a co-infection rate of 0.05%. Concordance between patient-reported history and serological findings was assessed, and occupational status, dental attendance, and family history were evaluated among seropositive patients. Conclusion: While seroprevalence rates were comparable to the general population, incomplete concordance between selfreported history and serological findings highlights the importance of preoperative screening and strict infection control.
