<p>OBJECTIVE: The azygos lobe (AL) is an anatomical variation observed in the upper lobe of the right lung, separated from the remaining lung tissue by the azygos fissure. This fissure contains the azygos vein, visceral, and parietal pleura. Awareness of this variation is crucial for anatomists and clinicians, as it is mostly asymptomatic but has been associated with anomalies such as intrapulmonary brachiocephalic vein, esophageal atresia, and extrapulmonary sequestration. In clinical practice, recognizing AL is essential for differential diagnosis and preoperative evaluations to prevent complications during thoracic procedures.MATERIAL AND METHODS: This retrospective study assessed the prevalence and classification of AL using chest radiographs of 24,494 patients who visited the pulmonary disease outpatient clinic of Samsun Training and Research Hospital between December 2022 and December 2024. Patient demographic data, including age and gender, were recorded, and AL types were classified as Type A, Type B, or Type C based on its relationship with the azygos vein.RESULTS: Among the 24,494 chest radiographs analyzed, AL was identified in 0.68% (n=167) of cases (male: 0.75%, n=89; female: 0.61%, n=78). The distribution of AL types was as follows: Type A (6%, n=10), Type B (41.3%, n=69), and Type C (52.7%, n=88). No significant difference was observed between genders in terms of AL type, though Type C was the most common in both males and females. The prevalence of AL in our study was higher than previous studies using chest radiographs. Studies using computed tomography have reported higher rates of AL detection, suggesting that some cases may be missed with standard chest X-rays.CONCLUSIONS: AL is an important anatomical variation that should be considered, particularly in preoperative evaluations. While it can be diagnosed using chest radiographs, computed tomography has a higher diagnostic value. AL may be associated with congenital malformations, emphasizing the need for careful radiological assessment. This study aims to raise awareness of AL and highlight the importance of meticulous preoperative imaging evaluations.</p></trans-abstract>
