                            <p>Objective: Celiac disease (CD) is a systemic inflammatory and multifactorial disease caused by intolerance to gluten. Genetic and environmental factors play a role in its development. The most important role in genetic factors is played by the HLADQ2.5 heterodimer formed by the HLA-DQB1*0201 and HLADQA1* 0501 alleles and the HLA-DQ8 heterodimer formed by the HLA-DQB1*0302 and HLA-DQA1*03 alleles. Our study aimed to determine the distribution of celiac disease susceptibility alleles and the diagnosis rate of celiac disease, in pediatric patients who underwent a genetic predisposition panel at Kırıkkale University Faculty of Medicine Hospital between January 1, 2018, and December 31, 2023. and also the distribution of HLA-DQ2/DQ8 in patients diagnosed with CD during this period. Material and Methods: Our study included 161 pediatric patients (71 male/90 female) In addition, genetic results were provided for 5 pediatric patients who were diagnosed with CD during the study period but were tested before the study process.Results: Among the pediatric patients who underwent CD panel testing, genetic susceptibility was detected in 139 (86.34%) patients; no susceptibility allele was detected in 22 (13.66%) patients. When the CD diagnosis was evaluated based on the genotypes of patients with detected susceptibility, 16 out of 62 patients (25.8%) with only HLA-DQ2 positivity, 2 out of 22 patients (9.09%) with only HLA-DQ8 positivity, 1 out of 7 patients (14.3%) with both HLA-DQ2 and HLA-DQ8 positivity, 1 out of 2 patients (50%) with HLA-DQ2 and HLADQB1* 0302 positivity, a patient with HLA-DQ2 and HLADQB1* 03 positivity (%100) and 2 out of 6 patients (33.3%) with HLA-DQ8 and HLA-DQA1*05 positivity received a diagnosis of CD. When the CD susceptibilities of the 161 patients were evaluated, 23 out of 103 patients (22.3%) with HLA-DQ2 and/or HLA-DQ8 positivity, and one patient with only HLA-DQA1*05 positivity and one patient with only HLADQA1* 03 positivity, in total 25 patients were diagnosed with CD. Conclusion: Among the 161 patients tested, 25 (15.53%) were diagnosed with CD, and HLA-DQ2 and/or HLA-DQ8 was detected in 93.3% of the patients diagnosed with CD. Our findings confirmed that HLA-DQ2 and HLA-DQ8 heterodimers contribute to susceptibility to CD, but they are not sufficient for diagnosis alone.</p></trans-abstract>
                                                                                                                                    <abstract><p>Amaç: Çölyak hastalığı (ÇH) glutene karşı intolerans nedeniyle oluşan, sistemik inflamatuvar ve multifaktöriyel bir hastalıktır. Gelişiminde genetik ve çevresel faktörler etkilidir. Genetik faktörlerde en önemli rolü HLA-DQB1*0201 ve HLADQA1* 0501 alellerinin oluşturduğu HLA-DQ2.5 heterodimeri ile HLA-DQB1*0302 ve HLA-DQA1*03 alellerinin oluşturduğu HLA-DQ8 heterodimeri oynamaktadır. Çalışmamızda Kırıkkale Üniversitesi Tıp Fakültesi Hastanesinde 1 Ocak 2018-31 Aralık 2023 tarihleri arasında ÇH genetik yatkınlık paneli çalışılan çocuk hastalardaki ÇH yatkınlık aleli dağılımı ile ÇH tanısı konma oranının saptanması, ayrıca ÇH tanısı alanlarda HLA-DQ2/DQ8 dağılımının belirlenmesi amaçlanmıştır. Gereç ve Yöntemler: Çalışma kapsamında 161 çocuk hasta (71 erkek/90 kız) değerlendirilmiştir. Ayrıca ÇH tanısını çalışma periyodunda alan ama yatkınlık paneli, çalışma süresinden önce çalışılan 5 çocuk hastanın da genetik sonucu sunulmuştur. Bulgular: ÇH genetik yatkınlık paneli bakılan çocuk hastaların 139’unda (%86,34) genetik yatkınlık saptanırken, 22’sinde (%13,66) herhangi bir yatkınlık aleli saptanmamıştır. Yatkınlık saptanan hastaların genotipleriyle ÇH tanısı değerlendirildiğinde; sadece HLA-DQ2 pozitif saptanan 62 hastanın 16’sı (%25,8), sadece HLA-DQ8 pozitif saptanan 22 hastanın 2’si (%9,09), HLA-DQ2 ve HLA-DQ8 pozitif saptanan 7 hastanın 1’i (%14,3), HLA-DQ2 ve HLA-DQB1*0302 pozitif saptanan 2 hastanın 1’i (%50), HLA-DQ2 ve HLA-DQB1*03 pozitif saptanan tek hasta (%100) ile HLA-DQ8 ve HLADQA1* 05 pozitif saptanan 6 hastanın 2’si (%33,3) ÇH tanısı almıştır. 161 hastanın ÇH tanısı alma oranları değerlendirildiğinde ise HLA-DQ2 ve veya HLA-DQ8 saptanan 103 hastanın 23’ü (%22,3) ile sadece HLA-DQA1*05 pozitif ve sadece HLA-DQA1*03 pozitif saptanan iki hasta olmak üzere toplam 25 hasta ÇH tanısı almıştır. Sonuç: Test yapılan 161 hastadan 25’ine (%15,53) ÇH tanısı konmuş ve ÇH tanısı konan hastaların %93,3’ünde HLA-DQ2 ve/veya HLA-DQ8 saptanmıştır. Elde ettiğimiz veriler, HLADQ2 ve HLA-DQ8 heterodimerlerinin ÇH’ye yatkınlık oluşturduğunu ancak tek başına tanı için yeterli olmadığını doğrulamıştır.</p></abstract>
